APPENDIX A NOTICE OF VIOLATION Virginia Electric and Power Company Docket Nos. 50-338 &'50-339 North Anna 1 and 2 License Nos. NPF-4 and NPF-7 As a result of the inspection conducted on September 6 - October 5,1981 and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980),
the following violation was identified.
Technical Specification 6.8.1 requires that written procedures shall be established and implemented. Abnomal procedure 1-AP-5.2 requires a daily stack sample and a recalculation of release rates prior to returning stack exhaust to nonnal whenever the vent stack monitor is in an alarm condition.
Contrary to the above, on September 30 and October 1, 1981, 1-AP-5.2 was not followed in that daily grab samples of the vent stack were not obtained and recalculated release rates for the affected system were not completed prior to restoring the discharge path to the environment.
This is a Severity Level V Violation (Supplement I.E.).
Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this office within thirty days of the date of this Notice, a written sta.tement or explanation in reply, including:
(1) admission or denial of the alleged viola-tion; (2) the reasons for the violation if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved.
Consideration may be given to extending your response time for good cause shown.
Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
The responses directed by this Notice are not subject to the clearance procedures of the Office of tianagement and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
