Subject: HRC Inspection of Washington Nuclear Projects 1 and 4
This refers to the inspection conducted by fiessrs P. P. Narbut and
G. Hernandez of this office on August 31 - September 4, and September 14-18,
1981 of activities authori7ed by NRC Construction Permit Nos. CPPR-134 and
CPPR-174, and to the discussion of our findings held with Mr. D. H. Mazur
and other members of your staff at the conclusion of the inspection.
Areas examined during this inspection are described in the enclosed
inspection report. Within these areas, the inspection consisted of
selective examinations of procedures and representative records,
interviews with personnel, and observations by the inspector.
Based on the results of this inspection, it appears that one of your
activities was not conducted in full compliance with NRC requirements,
as set forth in the Notice of Violation, enclosed herewith as Appendix A.
It appears that the quality perfomance of your heating and ventilation
contractor needs additional attention. Subsequent to the initial
allegations in January 1981 which resulted in six items of noncompliance,
!
additional allegations and inspections have determined that significant
quality problems are continuing. Consequently, in your reply you should
describe in particular those actions taken or planned to improve the
effectiveness of your management control system to detect and control
problems of this nature.

that you (cr your contractors) believe to be exempt from disclosure
under 10 CFR 9.5(a)(4), it is necessary that'you (a) notify this office
by telephone within ten (10) days from the date of this letter of your

intention to file a request for withholding; and (b)' submit within
25 days from the date of this, letter a written application to this
office to withhold such information.
If your receipt of this letter has

review, please notify this office promptly so that a new due date may be
established. Consistentwithsection2.790(b)(1),anysuchapplication
must be accompanied by an affidavit executed by the owner of the information
which identifies the document or part sought to be withheld, and which
contains a full statement of the basis on which it is-clained that the
infonnation should be withheld from public disclosure. This section
further requires the statement to address with specificity the considerations
