APPENDIX A NOTICE OF VIOLATIO?!

Washington Public Power Supply System Docket No. 50-460 P. O. Box 968 Construction Permit Richland, Washington 99352 fio. CPPR-134 As a result of the inspection conducted on August 31 - September 4 and September 14-18, 1981 and in accordance with the Interin Enforcement s

Policy, 45 FR 66754 (October 7,1980), the following violation was identified:

10 CFR 50, Appendix B, Criterion V, and Section 17.1.5. of the PSAR state in part:

"... activities affecting quality shall be prescribed by documented instructions, procedures, or drawings...and shall be accomplished in accordance with these instructions procedures, or drawings...."

UNSI procedure QCP/CP 10.0, " Nonconforming flaterials Parts and Components", paragraph 5.2.2. requires all nonconfonnances be dispositioned on the original nonconformance report and that the nonconformance receive engineering concurrence.

Contrary to the above on Septenber 3,1931 work on a defective tap-in duct piece was authorized by speednemo BLS-250 without benefit of engineering concurrence rather than having a nonconfonnance report generated and dispositioned by engineering.

This is a Severity Level V violation Supplement II, applicable to Unit 1.

Pursuant to the provisions of 10 CFR 2.201, Washington Public Power Supply System is hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:

(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further items of noncompliance; and (3) the date when full compliance will be achieved. Under the authority of Section-182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation. Consideration may be given to extending your response for good cause shown.

"The responses directed by this Notice are not subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511."
