Degradation of the reactor coolant pressure boundary and primary containment --

when the Isolation Condenser vent isolation valves (Y-14-1 and Y-14-19) for Is0Tacfon CchAwar "B" were saspected to have failed to close and also operation in a degraded mada as per Technical Specifications section 3.8.C.

This event is considered to be a reportable occurrence as defined fa the Technical Speci fh.atiores, paragraph 5.9.2.a.(3) and 6.9.2.b.(2).

Conditions Frfor to Occurrence 1he 74 asst m, aparating at steady state power.
