SUBJECT: NOTICE OF VIOLATION
(NRC INSPECTION REPORT NOS. 50-321/93-06 AND 50-366/93-06)
This refers to the inspection conducted by L. Wert of this office on
April 11 - May 8, 1993.
Tl.e inspection included a review of activities
authorized for your Hatch facility. At the conclusion of the inspection, the
findings were discussed with those members of your staff identified in the
enclosed report.
Areas examined during the inspection are identified in the report. Within-
these areas, the inspection consisted of selective examinations of procedures
and representative records, interviews with personnel, and observation of
activities in progress.
Based on the results of this inspection, certain of your activities appeared
to be in violation of NRC requirements, as specified in the enclosed Notice of
Violation (Notice).
Violation A in the enclosed Notice involved a loss of
shutdown cooling. Although the safety significance of this particular event
was small, due to the minimal decay heat load and the availability of other
cooling systems, we are concerned due to the length of time before the problem
was identified. Moreover, if this event had occurred under certain plant
conditions which existed earlier in the outage, a more significant safety
concern could have resulted. As part of your response to this violation,
please describe what actions, if any, are being taken to address overall_
!
awareness and responsibilities of the licensed operators and other plant
workers. Additionally, because the present Unit 1 Technical Specifications do
not contain sufficient require:nents regarding inoperability of shutdown
cooling, please inform us if you plan to submit a Technical Specification
amendment.
In addition, the enclosed Inspection Report also identifies a dosimetry
'
technician who falsified records. This item will not be the subject of
enforcement actions. The basis for this discretion was your prompt
identification, investigation, and correction of this willful violation, which
met the criteria specified in Section VII.B of the Enforcement Policy.
You are required to respond to this 1.etter and should follow the instructions
specified in the enclosed Notice when preparing your response.

response, you should document the specific actions taken and any additional
actions you plan to prevent recurrence. After reviewing your response to this
Notice, including your proposed corrective actions and the results of future
inspections, the NRC will determine whether further NRC enforcement action is
necessary to ensure compliance with HRC regulatory requirements.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of
this letter and its enclosures will be placed in the NRC Public Document Room.
The responses directed by this letter,and the enclosed Notice are not subject
to the clearance procedures of the Office of Management and Budget as required
by the Paperwork Reduction Act of 1980, Pub.
L.~ No.96-511.
