APPENDIX A NOTICE OF VIOLATION Niagara Mohawk Power Corporation Docket No. 50-220 Nine Mile Point Unit 1 License No. DE R-63 During an NRC inspection conducted on March 22-26,1993, a violation of NRC 1
requirements was identified. In accordance with the " General Statement of Policy and Pmcedure for NRC Enforcement Actions",10 CFR Part 2, Appendix C (1992), the violation is listed below:
Technical Specification 6.11 requires, in pan, that procedures for radiation protection be prepared and adhered to for all operations involving personnel radiation exposure.
l Licensee procedure AP-3.3.3 requires, in part, that all radiation workers verify that l
l pmper radiation dosimetry is used in accordance with radiological postings.

Contrary to the above, on March 22 and 25,1993, radiation workers failed to verify that proper radiation dosimetry was used in accordance with the radiological postings.
l Specifically, on Marth 27 a worker was found in an area of the New Radwaste l
Building that was posui as a radiologically restricted area without his l
thennoluminescent dosimeter (TLD) and self-reading dosimeter @RD), and on March i
l 25, a worker was observed leaving the Turbine Building which was posted as a l
radiologically restricted area without his TLD or SRD. Radiologically restricted areas j
are posted requiring workers entering the areas to carry their assigned dosimetry. On March 22, the worker had left his dosimetry clipped to a radiological posting at the l
step-off pad on the 247' ek.ation of the Old Radwaste Building, and on March 25 the j
worker had left his dosimetry in the Turbine Building attached to a laboratory coat.
l This is a Severity Level IV violation (Supplement IV).
Under t.he authority of Section 182 of the Act,42 U.S.C. 2232, this response shall be l
submitted under oath or affirmation.
l Pursuant to the provisions c! 10 CFR 2.201, Niagara Mohawk Power Corporation is hereby required to s"bmit a written statement or explanation to the U.S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator, Region I, and a cop" to the NRC Resident inspector at the facility that is the subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice). -This reply should be clearly marked as a " Reply to a Notice of Violation" and should include for each violation: (1) the reason for the violation, or, if contested, the basis for disputing the viestion, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid funher violations, and (4) the date when full compliance wH1 he achieved.
