NOTICE OF VIOLATION

(NRC INSPECTION REPORT NOS. 50-348/93-04 AND 50-364/93-04)

This refers to the inspection conducted by G. F. Maxwell of this office on

February 10 through March 22, 1993.

The inspection included a review of

activities authorized for your Farley facility. At the conclusion of the

inspection, and at various times during the inspection, findings were

discussed with members of your staff identified in the enclosed report.

Areas examined during the inspection are identified in the report. Within

these areas, the inspection consisted of selective examinations of procedures

and representative records, interviews with personnel, and observation of

activities in progress.

Based on results of this inspection, certain aspects of your activities

appeared to be in violation of NRC requirements as specified in the enclosed

Notice of Violation (Notice). The violations are of a concern to the NRC

because on February 2, with Unit 2 in a cold shutdown condition, surveillance

testing, which affected the boundary of a safety clearance, was performed on

emergency core cooling system components while maintenance was being performed

on a reactor coolant system pressurizer code safety valve. An operator error,

during the performance of the surveillance test, jeopardized the safety of

maintenance personnel and offered the potential for reactor coolant to be

released into the containment. Also, on February 5, with Unit 2 in a cold

shutdown condition, solid state protection system configuration was lost

during routine surveillance activities and, as a result, the unit experienced

an unexpected safety injection.

During a meeting held in the Region II Office on September 23, 1992, you

described your STAR concept (Stop, Think, Act, Review) which was implemented

in response to an adverse trend of personnel errors. While we noted the

program's initial effectiveness during the Unit I refuelirg outage

(September 25 through November 29,1992), we are concerned that these most

recent events may be indicative of a lessening of the effectiveness of these

corrective actions.

Please describe the actions you have taken or intend to

take to assure the continued success of this program in your response.

whether further NRC enforcement action is necessary to ensure compliance with

NRC regulatory requirements.
