NOTICE OF VIOLATION Arizona Public Service Co.

Docket No. 50-529 Palo Verde Unit 2 License No. NPF-51 During an NRC inspection conducted on December 15, 1992 through January 19, 1993, one violation of NRC requirements was identified.. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions,"

10 CFR Part 2, Appendix C, the violation is. listed b'elow:

10 CFR Part 50, Appendix B, Criterion V, requires that activities affecting quality be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances.

Surveillance Test Procedure 73ST-9ZZ18, Main Steam PSV Set Pressure Verification, Revision 4, PCN 3, states in Step 8.2.10 and a note.

following this step: "Reperform steps 8.2.7 through 8.2.9, until three (3) consecutive lifts have been performed within 1% of the-given set pressure of the valve. Record results on Appendix C, page 1 of 2.

NOTE In the event of an initial test failure, DO NOT 'make any adjustments to valve setpoint. After a minimum 15 minute [ sic] the~ test may be reperformed.

If results are-still unacceptable, adjustment 'of setpoint is necessary (after 2 or 3 ' successive out-of-tolerance lifts)', Section 8.3 steps shall be performed and verified in Appendix C, page -1 of 2."

Contrary to the above, as of October 8,1991 Test Procedure.73ST-9ZZ18, Revision 4, PCN 3, was not appropriate to the circumstances in that the procedure was not clear as to whether adjustment of a -safety valve was prohibited following the first failed test during the testing sequence, or only if the first test in the testing sequence failed.

This is a Severity Level V violation (Supplement I) applicable to Unit 2.

No response to this violation is required.
