SUBJECT:
NRC INSPECTION REPORT 50-445/92-60; S0-446/92-60 (NOTICE OF
VIOLATION)
This refers to the inspection conducted by Mr. D. N. Graves, and the
inspectors identified in the enclosed report, of this office on December 6,
1992, through January 30 1993.
The inspection included a review of
activities authorized for the Comanche Peak Steam Electric Station, Units 1
and 2, facility.
At the conclusion of the inspection, the findings were
discussed with you and those members of your staff identified in the enclosed
report.
Areas examined during the inspection are identified in the report.
Within
these areas the inspection consisted of selective examinations of procedures
and representative records, interviews with personnel, and observation of
activities in progress.
Based on the results of this inspection, certain licensed activities appeared
to be in violation of NRC requirements, as specified in the enclosed Notice of
Violation (Notice).
These violations are of concern because, collectively,
they indicate failures to effectively control system status, implement
corrective actions, and maintain the procedures that control the status and
operation of plant systems.
The lack of adequate system status control and
the use of inaccurate abnormal operating procedures could result in improper
or delayed operator actions.
It was also identified that a pipe support in the component cooling water
system was not grouted as required, and this was in violation of Criterion V
of Appendix B to 10 CFR Part 50. This violation is not being cited because
the criteria in paragraph VII.B.) of Appendix C to 10 CFR Part 2 of the NRC's
Rul es of Practice," were satisfied.
This violation was an isolated
occurrence and your staff took prompt and effective actions to correct the
condition.

It is our understanding, as discussed by Mr. J. Donahue, Manager, Operations,
in the exit meeting conducted on January 29, 1993, that the plant staff would
perform valve lineup verifications on all Unit 2 safety-related systems prior
to entering Mode S and field verifications of all Unit 2 and common abnormal
operating procedures prior to entering specified operating modes.
If your
understanding of these commitments differs from ours, please contact me
immediately.
In general, the transition of Unit 2 programs from the construction and
preoperational phase to operations was well controlled.
The preparation and
content of the open item closure packages provided to the NRC staff
significantly enhanced our ability to review and resolve issues in a timely
manner.
You are required to respond to this letter and should follow the instructions
specified in the onclosed Notice when preparing your response,
in your
response, you should document the specific actions taken and any additional
actions you plan to prevent recurrence.
After reviewing your response to this
Notice, including your proposed corrective actions and the results of future
inspections, the NRC will determine whether further NRC enforcement action is
necessary to ensure compliance with NRC regulatory requirements.
In accordance with 10 CFR Part 2.790 of the NRC's " Rules of Practice," a copy
of this letter and its enclosures will be placed in the NRC's Public Document
Room.
The responses directed by this letter and the enclosed Noti e are not subject
to the clearance procedures of the Office of Management and Budget, as
required b.y the Paperwor k Reduction Act of 1980, Pub. L. No. 96.511.
