This refers to the routine safety inspection conducte'd by Mr. D. Dempsey of.

this office on February 21 - April 2,1990 at Millstone Nuclear Power Station,:

.

Unit 1, Waterford, Connecticut of activities authorized by NRC License No.

' DPR-21, and to the discussions of our findings by Mr. Dempsey with Mr. J. -Stetz -

at the conclusion of the inspection.

0uring the inspection period,=our review indicated that since' December, 1976,.a

non-conservative main steam-line high flow setpoint had existed at-Unit 1,

t

which would have resulted in Group I main steam line isolation at.a steam flow

t

in excess of that required by technical specifications. Additionally, since at

least 1978, the emergency gas turbine generator had not been tested at' full;

load output.as' required by technical specifications.

In both' cases,. power

operation continued during these periods.in which applicable technical

i

specification limiting conditions for operation were not met'.

Both cases-

represent apparent violationr of the Commission's' regulations as detailed in-

the enclosed report.

In the case of the main steam line flow switches, we ~are c'oncerned .regarding

the lack of timely appreciation of and response to a significant-condition-

adverse to quality by unit management,

Regarding the gas turbine-generator event, we are' concerned that the procedure

review and approval mechanisms extant at Millstone.1 apparently were insufficient

to identify the inadequacy leading to this event.

The apparent violations identified in our review of these events are discussed:

in the enclosed inspection report. The need for, and nature of, appropriate

enforcement action concerning the apparent violations will be the subject of

separate correspondence at a later time.

These matters will be discussed in

an enforcement conference with you and' members of your staff scheduled .for
