During the weeks of: March 12 and March 19, 1990, the NRC administered requalification written, simulator, and walkthrough examinations to 28 employees of your company who hold licenses to operate the' South Texas Project j

nuclear facility. The purpose of these examinations was to evaluate the' a

effectiveness of STP's requalification training program to maintain the i

competency and currency of licensed operators as well'as satisfyingLthe requirement for renewal of licenses.of the individuals examined. -Two: reactor operators and two' senior reactor operators failed the written examination, and 4

the remaining individuals.and all crews evaluated were determined to be satisfactory.' The STP'requalification training program was evaluated as:

satisfactory in accordance with NUREG-1021

Operator performance on the plant walkthrough examinations was a noted' 1

strength. Also, the facility simulator demonstrated significant improvements in performance since the'NRC t %inistered examinations at STP in November 1988.

j Based on a review of the examination; test items submitted by the f acility. the' material was judged to be minimally acceptable, and-extensive. revision was required before it could be used on-this examination.

If;similar problems are-encountered during the next NRC requalification program evaluation,;the result 1

could be an unsatisfactory program evaluation; therefore, youlare encouraged to review and' to upgrade all examination items _ prior to your next requalification.

During the simulator examinations, inconsistencies were observed in the areas of control room communications. and of emergency operating procedure use and implementation. Similiar weaknesses were also observed during previous

-l examinations. You are encouraged to review the policies concerning control room comunications-and procedure usage and to consider whether changes are required to improve performance in this area.-

' Houston Lighting & Power Company Component labeling was observed to be'a problem. This has been the subject of previous inspection findings. However, the labeling problems were now observed affecting operator performance. Therefore, you may wish to review your conunitments in this area to assure that they will redress the problems noted with operator performance.

We also observed that your; procedures did not provide-sufficient guidance' to the operators for-the-operation of steam generator, power operated relief valves during a steam _ generator tube-leak or tube rupture. We understand that you intend to revise your procedures'to provide improved guidance in this: area.-

Additional details on these. items were provided to your staff and are included in the encicsed report. No response,to this letter is required.
