Inter-Agency Meeting With the Food And Drug Administration (FDA) l The NRC and FDA staff met on April 10, 1990, to discuss issues involved in the petition on the use of radiopharmaceuticals submitted by the Society of Nuclear Medicine and the American College of Nuclear Medicine. The FDA staff represented both the Center for Drug Evaluation and Research (CDER) and the Center for l
BiologicsEvaluationandResearch(CBER). The discussions involved the concerns that were raised by FDA recently on the NRC-developed interim final rule to amend 10 CFR Parts 30 and 35 and how these would be resolved. The FDA staff has agreed to provide suggested revisions to the rulemaking package by May 11 that I
should resolve the FDA concerns. The staff plans to hold a series of working sessions in the mean time and expects to finalize the interim final rule package that is mutually agreeable to both agencies by May 18.

Diablo Canyon Nuclear Power Plant. Units 1 and 2 At Region V's request, NRR has been evaluating the technical merits of a multi-part allegation regarding seismic adequacy of masonry walls at Diablo Canyon since September 1989. Region V kept lead responsibility for the allegation (RV-89-A-0042),andrequestedNRRtechnicalsupport. While several l
parts of the allegation have been resolved to the natisfaction of the NRR reviewer, several have not, and are being pursued with PG&E, Specifically, the staff is concerned that the licensee used the energy balance method to evaluate the masonry walls, with an allowed ductility ratio of 5.
The staff reviewer believes that a value of 3 is more appropriate, based on the available test data.
l Another major concern is the use of a six-inch-high, unreinforced, drypack concrete section to connect the top of the masonry walls to the_ ceiling, with clip angles that bear only on the drypack section. The energy balance technique depends on a positive connection at the top of the wall. The most recent staff requests for additional information on this subject were issued on March 20 and April 6, 1990. PG&E response is due by May 20, 1990.
The allegation originally came to the attention of Region V as a result of a lawsuit against PG&E filed by the alleger, a Bechtel structural engineer who l
had worked on the analysis of Diablo Canyon masonry walls. The alleger is seeking damages from PG&E for retaliation for bringing safety concerns about Diablo Canyon to the attention of PG&E management. The staff has had a number of telephone discussions with the alleger, who has cooperated fully, and is i
not requesting anonymity. Until recently, the alleger had pursued the issue in the courts, and in discussions with the staff.
The purpose of this highlight is to advise management that the alleger has decided to "go public" with his allegations. On. April 5,1990,thealleger's lawyer sent a letter to R. Zimmerman of Region V, reiterating his client s allegations and expressing his concern that NRC is permitting continued operation of Diablo Canyon, even though his client has informed the NRC of the alleged safety deficiencies at the plant. Copies of the letter were sent to members of the U.S. Congress, the press, the Sierra Club, and the California PUC; inquiries from some of these sources are possible.

2 Grand Gulf At about 2 p.m. on April 16, 1990, a public demonstration by local i
government officials and others was assembled outside of the gates of the Grand Gulf Nuclear Power Plant site.
The demonstration was in response l
to the restructuring of the licensee's (System Energy Resources, Inc.) tax payments made to Clairborne County, Mississippi. A restructuring would distribute taxes among the surrounding counties as a result of State legislative actions in 1986.
The governor of Mississippi has been briefed. Officers from the Sheriff's office were at the site. The fire and medical support from Vicksburg were alerted for standby. The licensee's securi+y staff augmented the shifts for extra coverage and the gates to the site were secured. The licensee also staffed the EOF and made a 10 CFR 50.72 report. At about 4 p.m., the demonstrators (estimated to number about 25) peacefully dispersed. The NRC security and emergency planning staff closely followed the demonstration and the licensee's response.
Peach Bottom. Units 2 and 3 As of April 18, 1990, startup activities were in progress for Unit 2 following extended activities to assure the operability of the Emergency Service Water l
(ESW) system. On March 21, 1990, the licensee reported that ESW flows to various Unit 2 components were inadequate under design basis conditions apparently due to silting and corrosion of ESW piping.
Licensee's corrective actions included inspection and cleaning of piping and associated heat exchangers, throttling of flow to the emergency diesel generators, and isolation of ESW flow to redundant ECCS room coolers. On Aaril 12, the licensee made a prrtentation to the NRC at the Region I offices on tiese activities. The unit was shutdown on March 2,1990 for a scheduled mid-cycle outage.

Unit 3 continues to operate at 100 percent power.
Following the annual shareholders meeting and a meeting of the Board of Directors on April 11, 1990, Philadelphia Electric Company announced a number of corporate management changes which were effective on April 16, 1990.
Corbin McNeill, Jr., formerly Executive Vice President-Nuclear, was elected President and Chief Operating Officer, and was named a Director of the Company.
Succeeding McNeill, Dickinson Smith was named Senior Vice President-Nuclear and will have responsibility for all the Company's nuclear operations. Donald Miller, currently station manager of the Connecticut Yankee Nuclear Station, was named to succeed Smith as Vice President for Peach Bottom.
In addition, David Helwig, formerly Vice President of Nuclear Services, moved to the newly created position of Vice President of Nuclear Engineering and Services following the retirement of Joseph Kowalski as Vice President of Nuclee.r Engineering.
Point Beach The Special Inspection Branch conduct.ed a pilot electrical distribution system functionalinspection(EDSFI)atthePointBeachNuclearPlantduringthe APRIL 20, 1990 ENCLOSVRE A
-3 period March 26 through April 6, 1990. The exit meeting was held with Wisconsin Electric Power Com)any (WEPCO) on April 17 at the utility's corporate offices in Milwaukee, W1. T1e team's major findings and the utility's reaction to the findings were discussed at the meeting. The inspection was difficult due to the lack of supporting engineering information.
The team was unable to determine whether the electrical distribution system
)
was able to function under all design conditions. The three issues that raised the most concern regarding operability were:
O emergency diesel generator loading that potentially can exceed the diesel generator ratings, O
nonseismic condition of portions of the emergency diesel generator fuel oil system, O
safety-related cables that were routed in raceway of the redundant division.
l WEFCO's reviews of these three items resulted in the plant entering limiting conditions for operations (LCOs) and making temporary modifications to the plant and emergency operating procedures.
The team's technical findings found weaknesses in the design and implementation of design for the electrical distribution system in three basic areas:
O The emergency diesel generator system and support systems primarily the potential to overload the diesels and the adequacy of fuel oil quality and the fuel oil transport system.

The plant's batteries and D.C. system high float voltage, ineffective surveillance practices, and lack of interrupting capability for the main bus circuit breakers.
O Potentials for single failure or common mode failures these included misrouted safety-related cables and single tie breakers between the redundant busses at both the 4160V and 480V level.
Damaged cable from a steam vent, uncontrolled relay calibration and maintenance work, and excessive cable tray fill were also identified.
Quad Cities Unit 2. Reactor Vessel Head Cracking During Quad Cities Unit 2's current refueling outage, many cracks were observed in the clad of the reactor vessel head located approximately 25 inches to 37 inches from the bottom of the flange. The licensee, Commonwealth Edison Co., utilized penetrant and ultrasonic examinations to characterize and size the cracks. To date, their examinations indicate that there are 41-flaws; 40 are circumferential1y oriented in the direction of welding and one is transversely oriented. The licensee's )reliminary examination results indicate that all flaws are located in t1e clad with the exception of 4 flaws that appear to extend through the clad into the base metal.

The licensee has removed four boat samples for metallurgical examination, and is excavating 10 flaws including 2 flaws that appear to extend into the base j
metal. The excavations and metallurgical examination will be utilized to determine the root cause of cracking and to confirm the ultrasonic test results.Section XI of the ASME Code does not require removal of flaws that are in the clad and do not extend into the base metal.

f Three Mile Island, Unit 2 The last anticipated shipment of fuel debris from TMI-2 left the site on i
Sunday, April 15, 1990. The shipment consisted of three rail casks containing a total of 20 canisters of fuel bearing material.
The staff is currently reviewing General Public Utilities Nuclear's (GPUN)
Defueling Completion Report. Thus far, the staff's review indicates that the licensee will be able to transition to Mode 2 on April 26, 1990 and to Mode 3 immediately afterward. Mode 2 recognizes the end of defueling and does not require licensed operators or borated water in the reactor coolant system.
Mode 3' recognizes the final offsite shipment and deletes borated water requirements from the spent fuel pool.
The evaporator which will be used to dispose of the 2.3 million gallons of accident generated water has passed the first of a series of 3 tests. A nonradioactive surrogate test solution of borated water was used. -On Monday, April 16 the evaporator testing arogram started a 12 week outage for further modifications to the solids landling and packaging system.
Testing is expected to resume in July.
Briefing With Commissioner Remick on General Electric's ABWR On April 16th staff members of the Division of Licensee Performance and Quality Evaluation briefed Commissioner Remick and members of his staff on human factors review of General Electric's Advanced Boiling Water Reactor (ABWR) design.
Issues discussed included the regulatory requirements and staff review criteria for the ABWR and, the staff's current review status of the ABWR.

Technical Specifications Improvement Program A meeting will be held between the staff and the NSSS Owners Groups working on i
thedevelopmentofnewStandardTechnicalSpecifications(STS). The purpose of the meeting is to discuss the review of the new STS Bases.
Vogtle Diesel Generator Mike Waterman, SICB, is at Vogtle to obtain information on the diesel generator temperature sensors that may have malfunctioned during the loss of AC power event. This visit is being coordinated with the IIT. The Yogtle licensee will have Wylie Labs conduct tests of the actual sensors in the near future to assist on establishing a root cause.
NRC staff are planning on witnessing these tests.

INDUSTRIAL AND MEDICAL NUCLEAR SAFETY Fuel Cycle Safety Dry Spent Fuel Storage Public Service Company of Colorado (PSC) staff has informed NMSS staff on thatPSCwillnotbesubmitting(alicenseapplicationfor April 16, 1990, dry spent fuel storage at PSC's Fort St. Vrain FSV)nuclearpowerplantsite.
An agreement has been reached by which the FSV gas spent reactor fuel will be shipped to Idaho beginning in August 1990.
UnitedNuclearCorporation(UNC),Montville, Connecticut On April 13, 1990, staff of the Office of Nuclear Material Safety and Safe-guards, Office of the General Counsel, and Region I met with UNC management representatives to discuss the proposed termination of activities at the fuel fabrication facility located at Montville, Connecticut. UNC's shutdown was announced on March 12, 1990, as a result of the Department of Energy's j
termination of future orders for the fabrication of naval nuclear fuel. During the meeting, UNC presented a schedule for the completion of decontamination l
of the facility by January 1992, and requested regulatory relief for certain-security upgrade requirements. UNC plans to again meet with the staff in May 1990, to review the technical aspects of the decontamination and l
decommissioning plan.
SAFEGUARDS AND TRANSPORTATION International International Atomic Energy Agency (IAEA) Inspection at the Babcock & Wilcox (B&W) Facility On April 12, 1990, an IAEA team initiated an announced inspection at the B&W l

June physical inventory of the low enriched uranium.
International Atomic Energy Agency (IAEA) Inspection at the General Electric Facility On April 17, 1990, an IAEA team commenced a short-notice random inspection at the General Electric Fuel Fabrication Plant in Wilmington, North Carolina. The team audited the low enriched uranium facility's ledger, weighed and measured the U-235 content by nondestructive assay of UF cylinders, and weighed and 6
sampled uranium dioxide powder.
Meeting with International Atomic Energy Agency (IAEA)
An International Safeguards Branch staff member met with IAEA representatives at the General Electric Fuel Fabrication Plant in Wilmington, North Carolina, APRIL 20, 1990 ENCLOSURE B

on April 10-11, 1990, to discuss issues related to differences between the operator's declared values of low enriched uranium scrap materials on inventory and the IAEA verification values. Problems were identified in the measurement and verification of certain categories of these heterogeneous materials.
Resolution of these differences was discussed and measures to be taken by the IAEA and the operator will be included in the physical inventory verification arrangements for the August 1990 physical inventory, to minimize the likelihood of such differences recurring.
Domestic Briefing for Department of Energy (DOE) Staff and its Contractors on NRC Licensing t
On April 17, 1990, Dr. Norton Haberman, Director, Division of Advanced Technology Projects and Technology Transfer, Office of Uranium Enrichment, Department of Energy, his staff and their contractors were briefed by the NRC staff on a typical NRC review process that might apply to licensing the proposed Atomic Vapor Laser Isotope Separation Process (AVLIS) uranium enrichment production plant.
If legislation is approved by Congress this year establishing a government corporation for this project, an application for a construction permit could be submitted to NRC as early as March 1991.
The NRC briefing covered areas including statutory considerations, the hearing process, fuel cycle safety licensing review process, security licensing review process, and safeguards considerations. The staff believes that the DOE personnel involved with the AVLIS project now have a better understanding of how a production facility license application is processed by the NRC and the timing of the review process.
Transportation Plutonium Air Shipment On April 12, 1990, theDivisionofSafeguardsandTransportation(SGTR) staff met with staff from Lawrence Livermore National Laboratory to review technical work being performed to support development of package testing criteria for plutonium air shipments, as required by the Mur(owski Amendment (Public Law 100-203, Section 5062). The discussion focused on draft controlled test criteria which would be conducted on a package as an alternative to conducting an actual air crash. Phase One of the test criteria development program will conclude with a draf t report in September 1990.
LOW-LEVEL WASTE MANAGEMENT Pre-Licensing Visits to Heritage Minerals. Inc. and Nord Ilmenite. Corp.
A pre-licensing visit was made on April 17 at Heritage Minerals, Inc. and Nord 11menite, Corp. by representatives from the Division of Low-Level Waste Management and Decommissioning, the Division of Industrial and Medical Nuclear Safety, and Region I to discuss NRC licensing requirements, collect material samples for analysis, and observe the sites and facilities.
Both facilities process mineral sand tailings from previous operations to extract heavy minerals, which also concentrates source material to levels above 0.05 percent by weight. Neither of APRIL 20, 1990 ENCLOSURE B
3 these companies currently have a Part 40 license for the possession of source material. However, both companies have submitted license applications to NRC and should be licensed in the near future.

The Incident Investigation Report of the source at Amersham is in final draft and is going to the printers on April 20, 1990.
The Incident Investigation of the Vogtle event is' proceeding on schedule. A briefing for the ED0 is scheduled for May 4 and the draft report is scheduled for May 8, 1990.

Office of Governmental and Public Affairs-Items of Interest Week Ending April 20. 1990 State Programs Completion of Texas Radiation Control Program Review The review of the Texas Radiation Control Program was completed this week in Austin, Texas. Preliminary staff findings will be reviewed by.
. Regional and Headquarters offices and the results of the review will be confirmed in a letter to the State.
Meetings with Dresden and Zion Residert Inspectors Roland Lickus, Chief,~ Region III State and Government Affairs, met with the NRC resident inspectors from the Zion Nuclear Plant on April-18,1990 and the Dresden Nuclear Power Station on April' 20, 1990. The purpose of-the meeting was to brief the resident inspectors on the Commission's policy on cooperation-with States,.the pending resident engineer subagreement with Illinois, and-the Illinois Department of Nuclear Safety (IDNS) plans to place State resident engineers at the two plants. The resident engineers could be stationed as early as May.
Region I Resident Inspector Meeting Carlton Kanonerer, Director, State Programs,- participated in a Region-I resident inspector meeting on April 19, 1990.in King of Prussia, Pennsylvania. Mr. Kammerer's comments-focused generally on NRC's program for interacting with States.

IBM 9370 Upgrade' The hardware to upgrade the IBM 9370 model 60 to a model 90 has been delivered by' IBM. The-installation.will start at 5:00 pm on Friday,-
c April 20, 1990 and continue through the weekend. The upgrade is expected to be complete and tested by Sunday, April 22. The users have been informed about the upgrade and the necessary downtime.

Acquisition of Additional Disk Space IRM received approval to proceed with'the acquisition of additional disk space for the leased Data General MV/20000 minicomputer. The disk space is necessary to install several of the old structured or " canned" searches from earlier DCS/NUDOCS system. The " canned" searches were formally requested by the Office of Nuclear Reactor Regulation in order to support their technical staff's document retrieval needs..

Week Ending April 20, 1990 Significant Enforcement Actions 1.
An Order Imposing Civil Penalty in the amount of $12,000 was issued on April 13, 1990 to Yale University, New Haven. Connecticut. The action was based on a reported overexposure of.178 rems to the finger of a researcher-
_l and 10 other violations indicative of a significant breakdown in management control over licensed activities.
The licensee responded to the Notice.in letters dated December 20,1989 and January 16, 1990. After. careful consideration of the licensee's response, the staff has concluded that the violations did occur as set forth-in the Notice of Violation and Proposed Imposition of Civil Penalties.

A Notice of Violation and Proposed Imposition of Civil Penalty-in the amount of $87,500 was issued on April 17, 1990 to Wisconsin Electric Power Company (Point Beach). The action was based on the licensee's failure to effectively implement a corrective action program that results in timely identification and correction of. deficiencies. About 75% of the open 1
items the licensee had identified and titled as "Significant Open Items,"
were either categorized as having no response or as having corrective actions overdue. The overdue actions involved various-safety-related i
systems and functions including AC and DC electrical systems, safety j
injection systems and-the radiological effluent system.
No immediate corrective action or evaluations were conducted for those deficiencies which may impact operability. The escalation and mitigation factors of the Enforcement Policy were considered and the penalty was escalated 50%
because NRC identified the deficiency and 25% because the initial correc-tive action was neither timely nor :dequate.

An Order Imposing Civil Penalty in the amount of $50,000 was issued on April 17, 1990 to Arkansas Power and Light Company (AN0). The action was based on a violation of the equipment qualification requirements of 10 CFR 50.49.
In accordance with the " Modified Enforcement Policy. Relating to 10 CFR 50.49," the violation was categorized as a Category B violation.
The licensee responded to the Notice in a letter dated June 22, 1989.
After careful consideration of the licensee's response, the staff has concluded that the violation did occur as set forth in the Notice of Violation and Proposed Imposition of Civil Penalty. While the staff does not agree with the licensee that its corrective actions warrant mitigation, it was concluded that the civil penalty should not have been escalated based on that factor. Therefore, the original proposed civil penalty has been reduced by $25,000 based on the licensee's arguments concerning corrective actions.

220% of the maximum permissible concentrations permitted by 10 CFR 20.106.

Bridgestone/Firestone Inc., Oklahoma City, Oklahoma, paid the civil penalty in the amount of $1,000. The action was based on incidents involving (1)lossofameasuringgaugecontaining25 mil 11curiesof cesium-137 and (2) entries into a vessel equipped with a radioactive gauge when the shutter on the gauge was not closed or locked.

The Cleveland Electric-Illuminating Company (Perry, Unit 1) paid the civil penalty in the amount of $100,000. The action was based on the licensee's failure to properly evaluate unsatisfactory scram time test results, failure to control nonconforming materials in the associated scram pilot valves, and failure to take appropriate corrective action for an identified scram time testing deficiency.

Recovery of Abandoned Radioactive Source A review by Region I to identify delinquent licensee correspondence, and subsequent-followup, revealed that a licensee was defunct and that the 1
only licensed material, an 1-125 source in a bone mineral cnalyzer (BMA),
had been abandoned. A Region I inspector located the BMA, with the source 4
still installed, in a locked storage room at facilities formerly occupied by the licensee, a McKees) ort, PA physician. - The current occupant of the facilities had a key to t1e storage room, but reported that they knew nothing of the contents and had not disturbed the BMA.
Further_
investigation revealed that the source was. manufactured by Nordion i
International, Inc. and that the current activity is approximately 0.1
~'
uCi. A Pittsburgh consultant who formerly did work for the licensee agreed to package and ship the source to Nordion, which has agreed to accept it.
2.
Oyster Creek At Oyster Creek Nuclear Generating Station,' the written and plant walk-through portions of an NRC licensed operator requalification examination were administered to 21 licensed operators during the weeks of April 9.and 16, 1990. Preliminary results indicate that the Oyster. Creek licensed operator requalification program will be declared unsatisfactory due to less than 75% of the operators passing the exam. The exam results evaluation by the NRC examiners indicates that 5 operators failed the written exam and 2 other operators failed the plant walk-through portion i
of the exam. The facility is not in agreement with the NRC results evaluation. An exit meeting is scheduled to take place in the Region I office on April 24, 1990. The simulator portion of the exam is scheduled i
to be conducted during May and June 1990-at the Nine Mile Point No. I simulator.

Fitzpatrick I
The licensee scheduled 41 welds for NUREG-0313 Rev. 2/ Generic Letter 88-01 examinations during the ongoing outage..As of April 19, 1990, 32 weld examinations were completed, 2 welds were classified as containing intergranular stress corrosion cracks and a third was-being evaluated.
The licensee is in the process of repairing the cracked welds using the weld overlay repair technique, and has discussed the repair process with the staff at NRR.
Based on the results.to date, the licensee has expanded the original examination sample by 14 additional welds, of which 3 were examined and 4
classified as being free of cracks.

South Carolina Electric and Gas Company On April 16, the Regional Administrator met with 0. Dixon, Executive Vice President, Operations, South Carolina Electric and Gas Company, to discuss issues pertaining to the Summer facility.

Duke Power Company On April 16, a Division of Reactor Projects Branch Chief and the Oconee Resident' Inspectors met with Pickens County local officials on the NRC's role at the Oconee Nuclear Station.

Industrial NDT (INDT) Company, Inc.
An Enforcement Conference was held-on April.17 1990, with the president ofIndustrialNDTCompany,Inc.,andmembersofhisstafftodiscuss inspection findings concerning their radiographic operations.. INDT's response to a source disconnect incident in Woodland, Maine was considered adequate although violations were.still cited.
An incident involving the potential for significant exposure of a radio-grapher who failed to lock a radiography camera while working inside INDT's radiography cull in Richmond, VA was also discussed.

RSI Status Meeting L
A meeting was held on April 19, 1990, in Decatur, GA to discuss the current status of the RSI recovery operation. Special form capsule-shipments were completed during the week of the meeting and normal. form capsule shipments were scheduled to begin during the. week. Of the.252 capsules originally present on site,167 or 66 percent of the capsules have been shipped.
No conclusions have yet been reached concerning the 4
mechanism /cause of the capsule failure that produced the original problem i
at RSI. Decontamination of the facility still stands at approximately 30 percent complete.
RSI expressed concern about the-length of time the recovery process was taking. The next. status update meeting is scheduled for mid-July at the PNL facility in Richland, WA.

Grand Gulf -Public Demonstration On April 16, 1990, a peaceful, public demonstration occurred at the Grand 1
Gulf facility. The demonstration was in response to State legislative action which restructured System Energy Resources, Inc.'s (Grand Gulf operator) tax payment made to Claiborne County and surrounding counties.
Anticipating the demonstration, the licensee made contingency plans with the City of Vicksburg and-the State of Mississippi for fire, police, and medical support should the City of Port.Gibson elect not to provide offsite emergency-support.
The demonstration consisted of approximately 25 local officials and-citizens from Claiborne County blocking the Grand Gulf north entrance. gate which had already'been secured by the licensee. The protesters left without incident after approximately one hour.
Media coverage was limited.
The Resident Inspector as well as Regional security and emergency prepared-ness specialists followed the licensee's activities. No further actions are anticipated.

Extensive ins)ection effort has continued into the brachytherapy treatment i
programs at tiese two hospital programs.
Based on inspections by NRC. staff-and a medical consultant, additional Confirmatory Action. Letters were issued which suspended all brachytherapy treatment at two hospitals under St. Mary Medical Center and brachytherapy treatment by one authorized user-at Porter Hospital. -Enforcement Conferences are scheduled regarding these i
matters during the week of April 23, 1990. Formal enforcement. action is under review by the. staff.

Braidwood and Byron Nuclear Power Stations.
The emergency diesel generators.(EDGs) at the Braidwood and Byron 4
stations have experienced several speed oscillations that have occurred between April 4-16, 1990. The licensee has determined that-the root cause of the EDG failures was due to failed dropping resistors in the governor.
H speed control circuit of the EDGs. As~of April.20, 1990, both' resistors were replaced in each EDG at the Byron and Braidwood stations.
The station management at each facility has acknowledged the possible generic implications and contacted the EDG supplier (Cooper Bessemer) and the governor manufacturer (Woodward) for 10 CFR 21 applicability and reporting requirements.
4.
Clinton Nuclear Power Station Following the April 11, 1990, inadvertent control rod withdrawal event, the licensee plans to restart on April 21, 1990. Region III implemented an augmented control room coverage plan to monitor the.startup to 35 percent power. The NRR Project Manager will assist with the coverage. The special inspection team plans to exit with the-licensee on April 25.

Callaway Nuclear Plant On April 19, 1990, A.' Bert Davis, Regional Administrator, Edward G. Greenman, Director, Division of Reactor Projects, and members of the Region III staff met with representatives of Union Electric Company in a public meeting to discuss the SALP 9 report. The licensee received seven Category 1 ratings during the period from September 1, 1988, through January 31, 1990.

Region III met with Detroit Edison Company management representatives in the Region III office.on April 16, 1990, to discuss vibration issues related to small bore piping on the RHR-system.. Pipe cracking and leaks have been observed and continued operability of the system is being evaluated.

Quad Cities Nuclear Power Station i

'On April vessel flooded,_an throughout the reactor building. The source of the spills was found to be the Unit 2 fuel pool-overflowing into the ventilation ducts. The licensee had lined up control rod drive (CRD) pump suction =from the. fuel pool-l" shutting normal suction from the condensate-storage tank.(CST).and was discharging the 40 gpm CR0 pump flow through the CRDs;back intu the fuel pool.
A. condensate hotwell test being performed simultaneously required condensate to be rejected back to the CST through the same header used by the CRD pump normal suction. Since the CST was isolated, the condensate
'hotwell_ reject backflowed.into the fuel pool causing it to overfill. -No personnel were contaminated, but area contamination levels ranged from approximately 10,000 to 20,000 disintegrations per minute (dpm) per 100 square centimeters with one area-reading 60,000 dpm. The licensee'is continuing cleanup.

Public Service Company of Colorado Enforcement Conference An enforcement conference was held on April 16, 1990, in Region IV with l
Public Service Company of Colorado to review the findings of an inspection conducted at the. licensee's facility during the period February 26 -
March 2, 1990. The NRC inspector examined the operational status of the emergency preparedness program and identified an apparent repeat violation

related to the inadequate emergency response training provided to control room personnel.

URF0 Meeting with Homestake Mining Company The Director, Deputy Director, and selected staff of the Uranium _ Recovery Field Office met with representatives of Homestake Mining Company on a
Aprii 45,.1990, tc discuss the possible disposal of several types of waste-generated at Homestake's Pitch Mine'in Colorado at the Milan Mill in New Mexico. The wastes' result from activities conducted under a radioactive materials license issued by the State of Colorado.

'Welf Creek Nuclear Generating Station The following information was obtained from a newspaper article that appeared in the April 14, 1990, Topeka Capital-Journal:
The Kansas Supreme Court, on April 13, overturned a lower court decision l-and freed anti-nuclear activists to disclose information obtained'from L
confidential files about the. Wolf Creek Nuclear Generating-Station. The information in question is copies of 68-Quality First files which contain L
information on employee concerns brought to the licensee's attention for resolution.
In a 4-2 decision, the Court said that the confidentiality of the files was not protected by federal copyright laws and that the files contained 4
information in which the public had "an overriding interest" and a right to learn about.
Kansas Gas & Electric Company (KG&E), the lead utility in building Wolf I
Creek, filed suit against the group with the files in 1985 to prevent them from disclosing any of the information in the files they obtained.
KG&E also sought the return of the files and a permanent injunction to block disclosure of information in the files. A district judge found in favor of KG&E, and the activist group appealed.
The activist group stated that-they would continue to protect the identity of plant workers whose comments made up the file of confidential documents.

Physicians' Laboratory Service Inc.
An enforcement conference was conducted with the-licensee at the Region IV Uranium Recovery Field Office in Golden, Colorado, on i
April 19, 1990. The conference addressed the findings related to an inspection conducted on March 20,-1990, and_ focused on'the lack of-management oversight of the radiation safety program resulting in a number of violations identified _during this inspection._ One of the violations identified indicated that Physicians' Laboratory Service,
-Inc., had performed activities in two unauthorized locations.

'URF0 Meeting with UMETCO Management of UMETCO met with URF0 staff in the Denver office on April 19, 1990, to discuss outstanding ground-water issues at the UMETCO Gas Hills site. Topics discussed included background water quality, effective area of the corrective action program,'and ground-water remediation.

URF0 Meeting with Homestake The Regional Manager of Environmental Affairs for Homestake Mining Company met with the Director of the Uranium Recovery Field Office in Denver on April 19, 1990, to discuss recent personnel changes and operational plans for the Homestake Mill.near Milan, New Mexico.

Conference with Barnett Industrial X-Ray A management conference was conducted Friday, April 20, 1990, in the Region IV office with members of Barnett Industrial X-Ray's management staff to review the licensee's evaluation of an incident that occurred on Friday, April 6, 1990, which resulted in the overexposure of an assistant-t I
radiographer while conducting licensed activities at a temporary. jobsite in Oklahoma. This discussion included a review of corrective actions which have been implemented since the incident. A Confirmatory Action Letter was issued on April 9, 1990, documenting the licensee's commitment to temporarily suspend licensed activities pending further review by NRC.-
An inspection was initiated on April 7, 1990, to review the circumstances surrounding the incident and the licensee's radiation safety program.
Regional staff has continued it's inquiry regarding medical evaluation of the assistant radiographer with further enforcement action pending..

Rancho Seco Nuclear Generating Station Recent newspaper articles have indicated that-the SMUD Board of Directors have narrowed the candidates for the position of General Manager to two people Mr. Dnvid Freeman, former head.of TVA, and Mr. Norman Nichols.
General Manager of the Los Angeles Department of Water and Power. The SMUD Board will be meeting Friday, April 20, 1990, to further discuss the candidates.
