Gentlemen:

This refers to the special safety inspection conducted by Messrs.

J. A. Hopkins, T. M. Tongue, and T. E. Taylor of this office from March 20

through April 5, 1990, at the Braidwood_ Station, Unit 2, authorized by NRC

Operating License No. NPF-77, and to the discussion of our findings with-

Mr. Querio and others of your staff at the conclusion of the inspection.

The special inspection was related to the inadvertent depressurization

of the reactor coolant system and rapid pressurizer cooldown event that.

occurred on March 18, 1990.

The enclosed copy of our inspection resort identifies areas examined during

the inspection. Within these areas, t1e inspection consisted of a- selective

examination of procedures and representative records, observations, and

interviews with personnel.

No violations of NRC requirements were identified during the course of this

inspection. The cause of the event appears to have originated as,a result

of poor communication practices between supervisors and operators. This

deficiency is being corrected by licensee initiated programs which will

identify non-routine evolutions that require increased communications

between supervisors and operators.

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy. of

this letter and the enclosed inspection report will be placed in the NRC.
