Subject: NRC Region I Inspection Report No. 50-286/90-01
This letter transmits the findings of the routine inspection conducted by
Messrs. -Lawrence W. Rossbach and Gordon K. Hunegs from January 9, 1990 to
February 26, 1990, at the Indian Point 3 Nuclear Power Plant, Buchanan, New
,
York.- The findings were discussed with you and yoer staff at the conclusion of
e
the inspection.
The inspectors concluded that procedures pertaining to alarm response and log-
keeping requirements were not followed and therefore NRC requirements were
-
violated. as set forth in the Notice of Violation, enclosed herewith as
Appendix A.
Similar weaknesses were identified in our Inspection Reports 89-06
and 89-25.
Although you have expressed to us your commitment to procedural
adherence, this- violation indicates continued weakness in this . area.
This
'
failure to follow the alarm response procedure (ARP) is of particular concern
because of the apparent operator and supervisor disregard of the ARP.
You are required to respond to this letter. We appreciate yc,ur cooperation.
