1 operating nuclear power plants. licensed by the NRC. These meetings are con-ducted biannually to focus NRC. resources co those plants.and issues of greatest-safety significance The NRC notes that Pilgrim, which was previously cate-1
_.porizedasrequirinhsubstantial-. improvement,hasbeensuccessfullyrestarted-j and_ operational performance.to date has been generally satisfactory. With.
this improvement, Pilgrim is~now categorized as requiring-close monitoring.
y Plants in this-category continue to receive increased NRC attention from both
' headquarters and the regional office.
t A summary of NRC discussions held relating to Pilgrim is provided below:

- The licensee was authorized on December 30, 1988, to start Up the plant and-
' operate up to 5% power.; Testing through 5% power was completed on:

February 16, 1989.. Equipment problems, which delayed testing, were handled s
1 well. Operations up to 5%' power-were performed safely and.in a conservative manner.- On March 3 1989,-the licensee'was authorized to operate at power
-levelsupto25Lfullpower.

Hardware failures and personnel errors during operation in the 5-25% range were discussed. The Reactor Core Isolation Cooling overpressure event of had multipic' personnel performance,causes and was the subject April 12,1989, of an Augmented Inspection Team review.--Concern was expressed., based upon the April 12, 1989 event, that some plant employees had not followed establish'ed administrative procedures.
It was noted that the licensee took strong corrective action as a result. The May-3, 1989 automatic.
- reactor shutdown was caused by inappropriately controlled troubleshooting g
.on'a feedwater regulating valve and was complicated by a partial Contain-j

ment Isolation System activation resulting from voltage transients which J
occurred after the. trip. These events occurred after the licensee had reduced its augmented management oversight on-shift. Licensee response, l
root cause analysis, and followup to the events were exceptionally good and demonstrated management's ability to perform indepth analysis of Nonetheless, the staff is concerned that these events may problems.
