Dear Mr. O'Reilly:
The Brunswick St'eam Electric Plant (BSEP) has received IE Inspection Report 50-324/81-24 and 50-325/81-24 and finds it does not contain any information of a proprietary nature.
The report identified one item that appears to be in non-compliance with NRC requirements. This item and Carolina Power & Light Company's (CP&L) response is addressed in the following taxt:
VIOLATION (Severity Level IV)
Technical Specifications 3.3.2 and 3.3.2.b requires that the isointion actuation instrumentation channels shown in Table 3.3.2.1 shall be OPERABLE, or place the inoperable channel in the tripped condition within one hour, or take the Action required by Table 3.3.2-1.
Contrary to the above, the inoperable channel was not placed in the tripped mode within one hour in that on August 27, 1981, while performing a periodic test on the HPCI temperature switch, it was determined that this instrument could not be calibrated. This event was identified at 2:00 p m. and the HPCI system ias not isolated and declared inoperable until approximately 7:15 p.m. on August 27, 1981.
CAROLINA POWER & LICHT COMPfff'S RESPONSE Carolina Power & Light Company acknowledges that this was a violation of NRC requirements. While performing surveillance testing on the HPCI and RCIC steam line temperature switches, it was discovered that the HPCI steam line area differential temperature switch E51-dTS-N604D could not be cali-brated. Shift Foreman to have it assigned a number. The Shift Foreman assigned a

number to the trouble ticket; however, no relationship between the inoperable l
instrument and HPCI operability was recognized at that time. The techni-l cian did not inform the operator that the testing was secured until the instrument was repaired; however, he did leave the instrument in test to j
j indicate the HPCI was out of service until the temperature instrument could be repaired and.the surveillance testing completed.
At 1530 hours0.0177 days <br />0.425 hours <br />0.00253 weeks <br />5.82165e-4 months <br /> during shift turnover, the oncoming Shift Foreman and operator noted that HPCI was in test and assumed that the surveillance testing was still in progress. At approximately 1915 hours0.0222 days <br />0.532 hours <br />0.00317 weeks <br />7.286575e-4 months <br />, while reviewing and processing trouble tickets not processed by the_ day shift, l
the Shift Foreman found the trouble ticket on E51-dTS-N604D. Following a review of Technical Specifications, the HPCI system was immediately isolated and declared inoperable.
The key issue'in this event was the failure of the shift foreman to i
promptly review the trouble ticket prepared as a result of the surveillance testing. The problem was compounded by the technician leaving the system in the test mode af ter the equipment problem was discoverca. All SR0s l
will be counseled concerning the necessity of promptly screening trouble tickets to_ identify those items requiring irmediate attention.
In addition the I&C periodic testing personnel will be counseled not to leave the l
equipment in test when the testing is halted. These actions will be com-pleted by November 13, 1981.
