A BELLEFONTE NUCLEAR PLANT UNITS 1 AND 2 - CERTIFIED MATERIAL TEST REPORTS ON ANCHORHEADS - NCR BLN CEB 8008 - FINAL REPORT The subject deficiency was initially reported to NRC-0IE Inspector R. W. Wright on November 26, 1980, in accordance with 10 CFR 50.55(e).

This was followed by our interim reports dated December 24, 1980 and March 26, 1981. Enclosed is our final report.

FINAL REPORT Description of Deficiency lI Certified Material Test Reports (CMTR's) have not been provided for several heats of tendon anchorhead material used in the prestressing system for the primary containment. American Concrete Institute (ACI) 359, April 1973 version, subsection CC-2131.1 and TVA contract specification require CMTR's for prestressing system materials. These reports must be provided by the materials manufacturer and shall include, in part, certified reports of analyses, physical tests, mechanical tests, and examinations performed on the material.

Inryco, Incorporated is the contractor for the prestreasing system (contract No. 75C53-85380) and supplies and installs the prestressing

-system materials. As a result of a July 22-23, 1980 TVA audit of Inryco, it was found that an Inryco steel supplier, Tech Steel and Alloy Corporation, is not a mill. Tech Steel receives material from various steel mills. The data appearing on Tech Steel's certificates is transferred to the certificates from the mill test reports. Inryco i

supplied the Tech Steel certificates to TVA to fulfill the requirements for CMTR's. These are Certificates of Compliance (C0C's), not CMTR's.

Further review of fabrication records revealed that another Inryco steel supplier, Scot Forge, is not a mill. Scot Forge receives material from i

various steel mills. The data appearing on Scot Forge's certificates is ji transferred to the certificates from the mill test reports. Inryco supplied the Scot Forge certificates to TVA to fulfill the requirements for CMTR's. These are Certificates of Compliance (C0C's), not CMTR's.

This deficiency was caused by Inryco's incorrect interpretation of the ACI 359 Code.

Safety Implication TVA has now received certified test reports on all anchorage material requiring them, thus fulfilling the TVA requiremento. As these are acceptable, had the condition remained uncorrected, it could not have affected adversely the safety of operations of the plant.

'.,e Corrective Action TVA requested Inryco to furnish CMTR's for heats numbered 91974,'97653, 81775, 52214, 6065780, and 6067474. These heats were supplied to Inryco by-the Tech Steel and Alloy Corporation. Because of difficulties between Inryco and Tech Steel and Alloy Corporation, delays were experienced in obtaining the CMTR's. TVA contacted Tech Steel and Alloy Corporation directly in an attempt to obtain the CMTR's. TVA then received several informal copies of the CMTR's. TVA has now received formal copies of CMTR's for heats numbered 91974. 97653, 81775, 52214, 6065780, and 6067474.

TVA also requested Inryco to provide a CMTR for heat numbered 219613 which was supplied to Inryco by Scot Forge. TVA received a copy of the CMTR along with a letter from the producing mill confirming the fabrication history.

To prevent a recurrence of this deficiency, Inryco has been informed again.

of the requirement for CMTR's (i.e., Inryce has been instructed that only a materials manufacturer can issue a CMTR). In addition, TVA inspectors were requested to assure that CMTR's were provided with documentation packages for anchorhead material. Before implementation of this request, one tendon was fabricated and released for shipment with anchorage material from heat numbered 53306. This heat was supplied by Tech Steel and Alloy Corporation and was accompanied only by a C0C. TVA has now received a formal copy of the CMTR for the heat numbered 53306.

All action required to correct this deficiency has been completed.

Bellefonte is the only TVA nuclear plant with a prestressed primary containment. Therefore no other TVA nuclear plants are affected.
