BELLEFONTE NUCLEAR PLANT UNITS 1 AND 2 - ASME CLASS I HANGER MATERIAL SUPPLIED BY ITT GRINNELL - BLRD-50-438/81-52, BLRD-50-439/81 REVISED FINAL REPORT The subject deficiency was initially reported to NRC-0IE Inspector R. V. Crlerjak on July 23, 1981 in accordance with 10 CFR 50.55(e) as NCR 1539. This was followed by our first interim report dated August 24, 1981 and our final report dated October 27, 1981. As discussed with R. V. Crlenjak on November 16, 1981, enclosed is our revised final report. This revision is necessary to clarify the report's corrective action section. We consider 10 CFR Part 21 applicable to this deficiency.

REVISED FIN 4L REPORT-Description of Deficiency TVA found " unacceptable" linear and rounded indications in the base material of some ASME Class I hanger material supplied to TVA from ITT I

Grinnell Corporation (ITTG), Providence, Rhode Island. This material is supplied in accordance with TVA contract 77K53-820732. The material was identified as M4 x 13 structural beam, heat number 80973, as manufactured by the Inland Steel Company, Chicago, Illinois, and 2 x 4 x 1/4" structural tube, heat number E90898, as manufactured by Welded Tube company of Americ.a, Philadelphia, Pennsylvania.

TVA used this material to fabricate ASME Class I hangers for the Makeup and Purification System. During routine dye penetrant examination of the TVA hanger welds, TVA found linear and rounded indications of the M4 x 13 base material adjacent to the TVA field weld in the radius of the beam. Linear indications were also found on the outside surface of the longitudinal weld seam of the structural tube.

The cause of the indications was determined by TVA and ITTG (through supplementary nondestructive examination and testing) to be the type of control used during the manufacturing process.

ITTG nondestructive testing (NDT) report No. 3057 has been issued to document the findings for the structural beams.

Safety Implications g

The results of the ITTG NDT of the structural beam show that the type of indications found in the base material are typical results of the rolling and extrusion processes. These type indications also typically occur in plates, pipes, and other components manufactured from these processes. The report also states that the indications are inconsequential and wvuld not affect the ability of the beam sections to perform their intended function.

Further, the beams were manufactured to specification ASME SA-36, and the report indicates they fully comply with the specification.

The indications found in the longitudinal weld seam of the structural tubing are considered defects by material specification ASTM A-500 (paragraph 9.0).

Hangers fabricated from this material might have failed to perform their intended design function.

Corrective Action In order to prevent delay, new beam sections were purchased before the completion of ITTG's testing. However, as stated above, the beam sections are adequate and will be used as is.. ITTG has agreed to more closely monitor the quality of all Class I material supplied to TVA.

ITTG has contacted the manufacturer of the structural tubing (Welded Tube Company of America) and informed them of the subject deficiency. Further inspection and testing of the tubing by ITTG and/or the manufacturer is considered by ITTG to be an effective means to prevent recurrence. TVA has replaced all the structural tubing (approximately 100 feet) with tubing that has been fully inspected and deemed acceptable and in accordance with the ma -erial specification.

In order to ensure no unacceptable material is used at Bellefonte, TVA continues to inspect all materials shipped onsite in accordance with Bellefonte Quality Control Procedure BNP-QCP 1.1, " Receiving Inspection,"

and will continue to report any significant deviations from code specifications per 10 CFR 50.55(e) should they occur.
