REPORT The subject deficiency was initially reported to NRC-0IE Inspector 1
R. V. Crlenjak on October 22, 1981 in accordance with 10 CFR 50.55(e) as NCR 1620. Enclosed is our first interim report. We expect to submit our next report by January 5, 1982. We consider 10 CFR Part 21 applicable to this deficiency.

FIRST INTERIM REPORT Description of Deficiency During torqueing _ operation in the field on' the motor operator adapter to q
the residual heat removal isolation valve (mark No. 3AWO412-ND-3 12-inch ASME III Class 2 motor-operated gate valve), a stud failed. Other studs on examination showed elongation. All studs had been torqued to 518 ft-lbs.
Borg Warner's drawing 80590 requires 500-550 ft-lbs.
Interim Progress Borg Warner was contacted. Their review of the problem showed that the torque value listed on drawing 80590 for the motor operator adapter to yoke.
connection studs was in error.
Borg Warner is to revise the drawing to show the correct torque value (120-140 ft-lbs). TVA will replace the studs using the newly afined torque values and during installation verify that tapped threads in the adapter are not damaged.
Additionally, Borg Warner has been reque sted to review all drawings / contracts supplied to TVA to determine if this situation exists elsewhere.
