BLRD-50-439/81 FOURTH INTERIM REPORT The subject deficiency was initially reported to NRC-0IE Inspector J. Crlenjak on February 27, 1981, in accordance with 10 CFR 50.55(e) as NCR 1390. This was followed by our interim reports dated March 31, June 3, and August 31, 1981. Enclosed is our fourth interim report. We expect to submit our next report by February 24, 1982.

FOURTH INTERIM REPORT

Description of Deficiency The discrepancy was discovered while chipping concrete, in accordance with disposition )f NCR 1308, to deter mine the locations of steel reinforcing bars at the main steam flued heaa cupports. Chipping revealed that these g

bars are not present between said supports. Additionally, it was discovered that additional reinforcing bars which should lace between the

- anchor plate support bars were cut off below the supports. After chipping further, it was found tnat the reinforcing descrioed above is missing for all anchors in valve room A for both units 1 and 2.

Interim Progress Upon reviewing the design, nonconformance report (NCR) BLN BLP 8120 was written against the design calculations. Vertical stiffening walls are being designed to resist the applied anchor loads, taking into account the missing rebar. NCR 1390 will be dispositioned after NCE BLN BLP 8120.
