.o APPENDIX A NOTICE OF VIOLATION Duquesne Light Company Docket No. 50-412 Beaver Valley Unit 2 License No. CPPR-105 As a result of the inspection conducted August 3-7, 17-21 and September 30, 1981 and in accordance with Interim Enforcement Policy 45 FR 66754 (October 7, 1980), the following violation was identified:
10CFR50, Appendix B, Criterion V states in part thati
" Activities affecting.
quality shall be prescribed by documented instructions, procedures... of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings."
Section 17.2.1.5A of Chapter 17 of the PSAR states in part that: "The Duquesne Light Company Qualicy Assurance Program requires that activitit,
affecting quality shall be prescribed by... procedures... appropriate to the circumstances and shall be acconplished in accordance with these
... procedures..."
Paragraph 5.1.2 of Stone and Webster Field Construction Procedure FCP 105 states in part; " Panels will be protected by covering with visqueen, plywood or other tarps to control dust..."
Contrary to the above, on August 6, 1981, grinding o'f welds was being performed inside the vertical control boards without adequate protection to keep dust and metal particles off electrical controls.
This is a Severity Level IV Violation (Supplement 11).
Although 10 CFR 2.201 requires you to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, we note that this 11.em of noncompliance was corrected prior to the completion of our inspection and your corrective steps taken and the results achieved as well as corrective steps taken to avoid further violations have been reviewed.
As a result, no response is necessary.
The responses directed by this Notice are not subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
