APPENDIX A NOTICE OF VIOLATION n

Mississippi Power & Light Co.

Docket No. 50-416 Grand Gulf License No. CPPR-118 As a result of the inspection conductea on July 27 - August 14, 1981, and in accordance with the Interim Enforcement volicy, 45 FR 66754 (October 7,1980),

the following violation was identified.

10 CFR 50, Appendix B, Criterion V and the implemented section of the Operational Quality Assurance !!anual, f1PL-Topical-1 paragraph 5.4.1 requires activities affecting quality be performed in accordance with documented instructions, procedures or drawings.

Grand Gulf Administrative Procedure 01-5-04-4 Revision 0, General Employee Traiaing (G.E.T) paragraph 6.7.4 states that the following topics will be covered in general radiation safety section of G.E.T.; decontamination of personnel and equipment, emergency actions relating to radiation accidents and housekeeping.

Paragraph 6.9 states that the minimum acceptable grade for satisfactory completion of the G.E.T.

program is 75%.

Contrary to the above, on August 4,1981 the following topics were not covered in general employee training, decontamination of personnel and equipment, emergency actions relating to radiation accidents and house-keeping. Three personnel were given credit for satisfactory, completing the G.E.T. program when their examination grades were less than 75%.

This is a Severity Level V Violation (Supplement II.E.).

Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:

(1) admission or denial of the alleged viola-tion; (2) the reasons for the violation if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will i

be taken to avoid further violations; and (5) the date when full compliance will j

j be achieved. Consideration may be given to extending your response time for good cause shown. Under the authority of Section 182 of the Atomic Energy Act of s

1954, as amended, this response shall be submitted under oath or affirmation.

of the Office of !!anagement and Budget as required by the Paperwork Reduction Act l
