Mr. John F. O' Leary, Director Re: Docket 50-155 Directorate of Licensing License DPR-6 US A*,omic Energy Con: mission Big Rock Point Wastdngton, DC 20545
Dear Mr. O' Leary:
By letter dated March 12, 1974, Consumers Power Company reported the failure of the Channel 3 neutron flux level instrumentation at the Big Rock Point Plant. The last paragraph of that letter misstated the corrective action that was taken. This misstatement was due to my mis-understanding of the inform.1 tion provided by the plant staff. The last paragraph should read:
" Bench testing of the failed a=plifier revealed a defective I
I series regulator tube (open filament) in the internal -150 V d-c I
power supply, resulting Ju co:nplete loss of the -150 volts and, l
therefore, fc.ilure of the indication and protective function of the channel. As this is the first known nonfail-safe failure of the amplifier's function of the picoazmeters at Big Rock Point, the manufacturer has been requested to analyze the failure and provide a recom:mendation for corrective c:casures
{
to prever.c recurrence. In addition, instructions have been issued to plant operations personnel explaining how to put the monitor in a trip condition if the pmblem recurs."
I I apologize for any inconvenience this error may have, caused.
r Yours very truly, l
I Ralph B. Sevell (Signed) l WGF/ map Ptlph B. Sewell Nuclear Licensing Administrator CC: James G. Keppler USAEC i
