This refers to the routine safety inspection conducted by Messrs.
J. R. Mullauer and W. P. Reichhold of tbis office on August 6 and 7, 1981, of activities authorized by NRC Byproduct Material Licenses No. 12-00780-04, No. 12-00780-05 and Special Nuclear Material License No. SNM-1784 and to the discussion of our findings with you, Mr. Larry Swearingen, Director, and members of your staff at the conclusion of the inspection.
The inspection was an examination of activities conducted under your license as they relate to radiation safety and to compliance with the Commission's rules and regulations and with the conditions of your license. The inspec-tion consisted of a selective examination of procedures and representative records, observations, independent measurements, and interviews with personnel.
This inspection also included a review of the actions described in your letter dated April 7, 1978, regarding the apparent items of noncompliance identified during our March 1, 1978, inspection. We have no further questions regarding this matter.
No items of noncompliance with NRC requirements were identified during the course of the inspection for License No. SNM-1784.
During the inspection of Licenses No. 12-00780-04 and No. 12-00780-05, it was found that certain of your activities appear to be in noncompliance with NRC requirements, as described in the enclosed Appendix A, and a written response submitted under oath or affirmation, is required.

i Blessing Hospital AUG 2df'tgg; This inspection also included a measurement of the output of your tele-therapy equipment. Our measurement was made using a Victoreen Model 570 Condenser R-Meter with Model 621 chamber, calibrated by the National Bureau of Standards Laboratory. The source-to-chamber distance was 80 centimeters and the field size was 10 x 10 centimeters. After applying-standard correction factors for temperature, pressure, timer error, and nine days of decay, our measurement of the output of your teletherapy equipment was 67.8 roentgens per minute. The August output of your teletherapy equipment, calculated from the February. 18, 1981, full cali-i bration reported a measured output for the same exposure parameters of 66.4 roentgens per minute.
i Based oe the close agreement in measured output values, we feel the actual output of your teletherapy equipment to be within established guidelines of 15 percent.
