As a result of the inspection conducted on August 6 and 7, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:
License No. 12-00780-04, 1.
10 CFR 35.21(b) states, full calibration measurements shall include determination of uniformity of the radiation field.
Contrary to the above requirement, it was determined through statements by licensee representatives that this condition is not being met.
Speci-fically, a licensee representative stated, uniformity of the radiation field is not checked during annual full calibration procedures.
This is a Severity Level V violation (Supplement VII).

10 CFR 35.25 states, the licensee shall maintain for inspection by the Commission, records of annual full calibrations, calibration of instru-ments used to make spot check measurements, and records of the licensee's evaluation of the qualified experts training and experience.
Contrary to the above requirements, it was determined through review of teletherapy records that this condition is not being met.
Speci-fically, the above records were not available for review on the day of this inspection.
This is Severity Level VI violation (Supplement VII).
License No. 12-00780-05 3.
License Conditica No. 14 requires that licensed material be possessed and used in accordance with statements, representations, and procedures contained in application dated April 30, 1976, May 31, 1977, and March 30, 1978; and letters dated May 28, 1976, January 21, 1977, and May 16, 1978.
The license application dated March 30, 1978, states, daily G-M surveys of the generator elution area will be performed and the results recorded.

taken to avoid further noncompliance; and (3) the date when full compliance will be achieved. Under the authority of Section 182 o' the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
Consideration may be given to extending your response time for good cause shown.
