LIMITORQUE VALVB OPERATORS 10C FR5 0. 5 5 ( e ), SIGNIFICANT DEFICIENCY NO. 1 AND 2 UNITS HOPE CREEK GENERATING STATION On June 19, 1981, a verbal report was made to Region I, Of-fice of Inspection and Enforcement representative, Mr. R. Gallo, advising of a potential significant deficiency concerning Limitorque Valve Operators. It was reported that an inspection of twenty (20) Limitorque valve operators was performed and the following specification nonconformances were observed: 1. Fiber shims under contact screws. 2. Unidentified terminal blocks. 3. Damaged terminal blocks. l l The following information is submitted as required by 10CFR50.55(e): 1. Description of the Deficiency: Our Architect / Engineer, Bechtel Power Corporation, re-ports the use of fiber shims under torque switch contact screws in Limitorque valve motor operators and the sub-sequent loosening of the screws has resulted in a re-portable deficiency in accordance with 10CFR50.55(e) on another Bechtel project. It has been determined that 219 Limitorque operators size SMB-00 and SMB-000 which contain fiber shims have been delivered to the Hope l Creek site. A survey of Limitorque operators for the ~ j other type deficiencies known to exist on other Bechtel l projects has been conducted and the results are being evaluated by Bechtel Engineering. 8107290157 810717 l PDR ADOCK 05000354 [ S PDR Si lp _

Analysis of Safety Implications: The required analysis concerning the reportable defic-iency (i.e., fiber shims) and the safety implications will be performed and reported to the NRC when complete. 3. Remedial Action: The corrective measures to be taken concerning the re-portable deficiency and those which may be required af ter review of the results of the survey for the other deficiencies noted at other Bechtel projects have not been determined. 4. Projected Completion Dates: a. The projected completion date for the snfety anal-ysis of the reportable deficiency concerning fiber shims and corrective action to be taken is August 28, 1981. b. Bechtel Engineering is evaluating the results of the survey for the other valve operator deficiencies. Any deficiencies which, if gone uncorrected, could have adversely af fected safe operation of the f acil- _ty will be identified. Scheduled completion date for this evaluation is September 8, 1981. Upon completion of the engineering analysis and the determ-ination of appropriate corrective action (s), a final report will be submitted to your office. Very truly yours,
