APPEND _IX A NOTICE OF VIOLATION Power Authority of the State of New York Docket No. 50-286 Indian Point Unit 3 Licensa No. DPR-64 As a result of the inspection conducted on December 16, 1932 - January 15, 1933, and in accordance with the NRC Enforcement Policy 47 FR 9987 (March 9,1982),

the following violation was identified:

The Quality Assurance Prngram Manual, Appendix A states:

" Coll 2c+1on,

storage and maintenance of Nuclear Power Plant Quality Assurance re-cords conforms with Regulatory Guide 1.88."

Regulatory Guide 1.83, Revision 2 of 1976 states:

"The requirements and guidelines for collection, storage, and maintenance of nuclear power plant quality assurance records that are indicated in AHSI H45-2.9-1974 are acceptable to the HRC staff, and provide an adequate basis for complying with tne pertinent quality assurance requirements of Appendix B to 10 CFR 50...."

The Station Administrative Prccedure AP-18, " Record Control and Storage" states:

"The purpose of the procedure is to define the responsibility for, and provide a method for the collection, filing, indexing, storage, maintenance, and disposition of records subject to the provisions of Regulatory Guide 1.88, Revision 2 of 1976 and ANSI N45-2.9-1974."

AASI H45-2-9-1974 states in part, "Uhere a single record storage facility is naintained, at least the follcwing features should be considered in its construction." Feature No. 8 requires an adequate fire protection system.

This ANSI further states in part, in Appendix A. Part A.6, " Operational Phase Activity Records" that a record of " Transient or operational cycling records for those plant components that have been designated to operate safely for a limited number of transients or operational cycles" must be maintained for the life of the plant.

The installed Halon Fire Protection system had been disconnected, 4

thus rendering the fire protection inoperable.

This is a Severity Level V Violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Power Authority of the State of New York, is hereby required to submit to this office within thirty days of the date of the letter transmitting this Notica, a written statement or explanation in reply, including:

(1) the corrective steps which have been taken and the re-sults achieved; corrective steps which will be taken to avoid further violations; and the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending your response time.
