APPENDIX A NPS Industries, Incorporated Docket No. 99900736/82-01 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on November 15-19, 1982, it appears that certain of your activities were not conducted in accordance with NRC requirements as indicated below:
Criterion V of Appendix B to 10 CFR Part 50 states:
" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.
Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."
Nonconformances with these requirements are as follows:
A.
Section 4.8 of Work Procedure No. 4.1.1B, " Material Release and Release Document Control (Welded Products)," Revision 1, dated August 3, 1981, states, in part, ".
. all plant records for the Material Release shall be maintained in QA records.
Contrary to the above, during the review of QA records relating to the shipment of items to the Texas Utilities Generating Company for the pipe whip restraint support structure, it was noted that the Gibbs & Hill Quality Assurance Release Form for shipment No. 4 was missing.
B.
Section 4.3 of Work Procedure No. 9.2.2, " Control of Welder Qualification," Revision 1, dated August 15, 1980, states, in part, "The Performance Qualification Record shall record.
. essential variables.
Contrary to the above, a review of qualification test records for welders revealed that the "F No.," an essential variable, was not recorded on the performance qualification records of four welders (identified by Nos. 78, 51, 87, and 81).

Paragraph 9.3.4a of Section 9.3, " Nondestructive Examination," of the Corporate Quality Assurance Manual, Revision 0, dated July 2,1982, states, in part, " Performance and acceptance of production NDE will be documented on the WDS.

Contrary to the above, a review of weld data ~ sheets (WDS) relating to weldments on the pipe whip restraint support structure' indicated that one inspector (identified by No. 47) performing magnetic particle examina-tion did not sign a WDS on five occasions, and another inspector

(identified by No. 2) performing ultrasonic examination did not sign a WDS on one occasion.
D.
Paragraph 12.0.4a of Section 12.0, " Control of Measuring and Test Equipment," of the Corporate Quality Assurance Manual, Revision 0, dated July 2, 1982, states, in part, "Each device shall be identifiea with unique marking traceable to its calibration record.
J Section 3.1 of Work Procedure No. 12.0.1, " Control of Measuring and Test Equipment," Revision 3, dated December 3,1981, states, in part, "Each device will be assigned a permanent identifying number by NPSI, which will be applied.
. with a durable tag.d Section 3.2.2 of Work Procedure No. 12.0.1 states, in part, " Devices shall be color coded.
to identify date for recalibration...."
Contrary to the above, an examination of the weld material storage area revealed that one dial stem thermometer (NPSI 223) on a holding oven had j
neither a tag with an identifying number nor a color coded marker to indicate the due date for the next calibration, and a second dial stem thermometer (NPSI 257) was missing the color coded marking.
E.
Paragraph 9.1.4e)2 of Section 9.1, " Welding Control," of the Corporate Quality Assurance Manual, Revision 0, dated July 2, 1982, states, in part, " Preheat and postweld heat treatment.
. shall be in accordance with the WPS."
Section 4.2 of AWS D1.1-80 states, in part:
. the minimum preheat and interpass temperatures shall be in accordance with Table 4.2...."
Table 4.2 requires that the minimum preheat and interpass temperature for ASTM A588 steel should be a minimum of 50 F for thickness of items 3/4" through 1-1/2".
Contrary to the above, a review of weld data sheets for the pipe whip restraint support structure revealed that two weldments (Weld Nos. W-10 and W-22 on the D-140-1-A East assembly) joining 7/8" angle to 1" plate did not specify any preheat or interpass temperatures.
