This refers to the routine safety inspection conducted by Dr. M. H. McBride of

this office on September 27 to October 1, 1982 of activities authorized by NRC

License No. DPR-35 and to the discussions of our findings held by Dr. McBride

with Mr. J. Ballentine, Vice President, Operations, Mr. C. Mathis and members

of your staff at the conclusion of the inspection.

Areas examined during this inspection are described in the NRC Region I Inspec-

tion Report which is enclosed with this letter. Within these areas, the

inspection consisted of select.ive examinations of procedures and representative

records, interviews with- personnel, and observations by the inspector.

While no violations were observed, the inspector identified an apparent weak-

ness in health physics management as indicated by lack of adequate response to

an incident involving decontamination activities at the Concentrator Room, on

September 10, 1982. Our findings indicated that health physics management had

not initiated a thorough review of the circumstances leading to:

1) the

inadvertent installation of a faulty portable ventilation unit for the Concen-

trator Room work and 2) the failure of health physics technicians to promptly

notify health physics management of a potentially significant uptake of radio-

(

active material by workers in the area. As a result of this inspection, a

corporate audit of the incident was conducted and identified several inadequacies.

'

We are concerned about the performance of your personnel which resulted in

this incident and the failure of plant management to promptly review and

initiate timely action to prevent the recurrence of such incidents.

Conse-

quently, in your reply, please describe those actions taken or planned to

improve the performance of your personnel in this area. Also, provide the

steps which have or will be taken to improve communication between site per-

sonnel, and to establish thresholds for notification of plant management of

changing radiological conditions at the plant.

Our inspector also verified the steps you had taken to correct the vio1 1tions

brought to your attention in letters dated March 9, 23 and July 7, 1982. We

have no questions in regard to these matters.

The response directed by this letter are not subject to the clearance pro-

cedures of the Office of Management and Budget as required by the Paperwork

Reduction Act of 1980, PL 96-5411.
