APPENDIX A NOTICE OF VIOLATION Texas Utilities Generating Company Docket:
50-445/82-30 Comanche Peak, Unit 1 Permit:
CPPR-126 Based on the results of an NRC inspection conducted during the period of December 13-16, 1982, and in accordance with the NRC Enforcement Policy (10 CFR Part 2, Appendix C), 47 FR 9987, aated March 9, 1982, the following violation was identified:
Failure to Follow Procedures Criterion V of Appendix B to 10 CFR Part 50 requires that activities affecting quality be accomplished in accordance with instructions and/or procedures.
Paragraph 3.2.B of Texas Utilities Service, Inc., Instruction CP-EI-4.5.4, Revision 3, " Technical Services Engineering Instruction for Pipe Hanger Design Review," requires that the design review be performed by the authorized rr.presentatives of the original design organization (ITT Grinnell Corporation and Nuclear Power Services, Inc. (NPSI)) in accord-ance with the respective design review programs.
Paragraph C.1.D.1 of Procedure QCES-2.3.6 of the ITT Grinnell Corporation Engineering Services QA Manual requires that when an error is detected, l
the engineer performing the check shall enter the correct value and draw a line through the incorrect value.
He shall then trace the impact of the correction throughout the preceding and remainder of the calculation i
package.
In addition, paragraph 3.2.3 of WP 3.1.5 of the NPSI Project Procedures Manual requires the checker to check the calculations for accuracy.
f Contrary to the above, the NRC inspector's review of calculation packages for pipe hangers / supports processed by both organizations disclosed mathematical errors that were not identified during the checking process.
These calculation packages were for the following hangers / supports:
SI-1-093-011-S42R FW-1-101-008-CS2S This is a Severity Level V Violation.

Pursuant to the provisions of 10 CFR 2.201, Texas Utilities Generating Company is hereby required to submit to this office, within 30 days of the date of this Notice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further vic1ations; and (3) the date when full compliance will be achieved.
Consideration may be given to extending your response time for good cause shown.
