Cantrell on August 24, 1982 in accordance with 10 CFR 50.55(e) as Audit 82V-29, Deficiencies 3 and 4.

THIRD INTERIM REPORT Description of Deficiencies During the TVA audit of GA Technologies, Inc. (formerly General Atomic Company), San Diego, California, the following deficiencies were noted in the implementation of their previously accepted QA program for TVA contracts:

Deficiency 3 Contrary to section 11, paragraph 5.3, of GA Technologies' QA manual, a test operator did not completely record the test results for a radiation monitor (TVA item 03513501-001) already shipped to TVA for use at Bellefonte.

Deficiency 4 Contrary to ANSI N45.2, section 18, which requires results of inspections to be identifiable and retrievable from records storage, QA records from GA Technologies' Cbmex facility do not provide for the required identification and retrievability of inspection results.

GA Technologies has also supplied radiation monitoring equipment for other TVA nuclear plants.

Interim Progress Concerning incomplete test results of previously shipped monitors, TVA has initiated a review of documentation packages for these monitors to assure that test results were properly recorded. For the monitor shipped to Bellfonte, GA Technologies has proposed field testing the item, and TVA is working with GA Technologies to schedule these field tests.

Concerning the QA records problem identified in deficiency 4 of this audit, GA Technologies has responded that routing travelers with applicable related documentation are now reviewed at three different levels, that the Cimex QC inspector has received documented training concerning QA record retention requirements, and that in serializing batches duplicate numbers are no longer used. TVA believes that these actions are sufficient to correct the deficiency, and plans to evaluate their implementation during the month of March 1983
