APPENDIX A NOTICE OF VIOLATION Public Service Electrh and Gas Company Docket No. 50-311 Salem Unit 2 License No. DPR-75 An inspection, conducted on October 6-12, 1982, detemined that an undetected degradation of the auxiliary feedwater system existed for about 7 hours8.101852e-5 days <br />0.00194 hours <br />1.157407e-5 weeks <br />2.6635e-6 months <br /> on October 4-5,1982, when valve 23AF23, auxiliary feedwater isolation valve to 23 steam generator, was closed and tagged instead of the intended valve, 23MS45.
The ermr went undetected because independent verification of the tagging and valve alignment operation, through a subsequent check by a second qualified individual, was not perfomed in accordance with appmved station procedures.
Themfom, in accordance with the NRC Enforcement Policy. (10 CFR 2, Appendix C) published in the Federal Register on March 9,1982 (47 FR 9987), the following violation of NRC mquirements is cited:

Technical Specification 6.8.1 requires the preparation and implementation of equipment control pmcedures. Administrative Procedum (AP)-15
" Safety Tagging Pmgram" mquires an independent verification of alignment and tagging when safety related equipment, including auxiliary feedwater, Request and Inquiry System)perations Directive (OD)-8, " TRIS (Tagging is taken out of serttce. O Tagging Operations" describes independent verification as a subsequent check of the tagging and valve alignment by a second qualified individual.
Contrary to the above, on October 4,1982, independent verification of a tagging and valve alignment operation on Auxiliary Feedwater Pump 23 was not conducted in that a subsequent check by a second qualified individual was not perfomed.
This is a Severity Level IV Violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, Public Service Electric and Gas Company is hemby requimd to submit to this office within thirty days of the date of the letter transmitting this Notice, a written statement or explanation in mply, including:)(1) the cormetive steps which have been taken and thethe correct l
results achieved; (2 violations;and(3)thedatewhenfullcompliancewillbeachieved. Where good cause is shown, consideration will be given to extending this msponse time.
