Report of Enforcement Conference Held November 9, 1982; Combined
Meeting Report No. 50-272/82-32 and 50-311/82-30
This refers to the subject Enforcement Conference held in the NRC Region I
office on November 9, 1982 and attended by you and members of your staff and
Mr. J. Allan, Deputy Regional Administrator and myself and other members of the
Region I staff. At this conference, we discussed an event which occurred on
October 4, 1982 invol'ing failure to follow station procedures in conducting
v
valve alignments and equipment tagging at Salem 2.
This event is discussed in
NRC Inspection Report 50-272/82-28, which was forwarded to you by our letter
dated October 26, 1982.
As a result of this event, an apparent violation of NRC requirements is cited
in the enclosed Appendix A and is categorized under the NRC Enforcement Policy,
10 CFR 2, Appendix C (47FR9987, March 9, 1982). A reply is required and 2.cald
be prepared in accordance with Appendix A.
It is exempt from the Office of
Management and Budget's clearance procedures under the Paperwork Reduction Act
of 1980, PL 96-511.
As discussed in the Enforcement Conference, we are concerned that this item
resulted from a failure to follow approved station procedures which were
developed following a series of similar occurrences.
Such failures to follow
proceduces reflect on the training and attitude of field personnel who are
charged with carrying out those procedures. These failures also raise concern
with regard to the effectiveness of supervision, adequacy of quality reviews
and the extent to which field personnel are held accountable for procedure
adherence. You are also requested to address these concerns in your written
response to Appendix A.
In accordance with 10 CFR 2.790(a), a copy of this letter and the enclosures
will be placed in the NRC Public Document Room unless you notify this office,
by telephone, within ten days of the date of this letter and submit written
application to withhold information contained therein within thirty days of the
date of this letter.
Such application must be consistent with the requirements
