IV. EVENT DESCRIPTION:

On December 1, 1982 at 0530 hours0.00613 days <br />0.147 hours <br />8.763227e-4 weeks <br />2.01665e-4 months <br /> a steam leak was discovered on a steam pressure sensing line for the RCIC System. The leak affected DPIS lE31-N007B and PS 1E31-N022B&D.

l V. PROBABLE CONSEQUENCES OF THE OCCURRENCE:

At the time of the occurrence the reactor was in the RUN mode producing 1474 MWT, 450 MWE.

l The containment outboard isolation valves for the affected switches, j 1B21-F414A&B, were immediately closed.

Redundant Differential Pressure Indicating Switch 1E31-N013B would have i performed its design function had a steam line isolation been required.

Redundant pressure switches 1E31-N022A&C would have isolated the RCIC Outboard Steam Line Isolation Valve on low pressure as designed. The health and safety of the public was preserved at all times.

VI. CAUSE:

The cause of the steam leak appeared to be the failure of a compression fitting on the instrumentation steam pressure line. The cause of the l

failure could not be determined. This was an isolated incident.

VII. CORRECTIVE ACTION:

The compression fitting was tightened immediately, however, the leak was not curtailed.

The outboard containment isolation valves, 1B21-F414A&B, were then closed to isolate the instruments. The RCIC Steam Supply Isolation Valves 1E51-F063 and 1E51-F008 were also closed.

Work Request L20835 was written to repair the tubing. This Work Request was completed and the pressure switches were returned to service on 12/1/82. The RCIC System was tested satisfactorily on 12/20/82 per

(. LOS-RI-Q3 following a 17 day outage.
