EVENT DESCRIPTION

On October 19, 1982, a small leak was discovered in the socket weld of the mechanical seal injection line valve for condensate / condensate booster pump "l A".

The shift ordered "lC" CD/CB pump started and shutdown "1A" CD/CB pump. The leak was isolated and all leakage was contained within the pump room and routed to Radwaste via floor drains.

Work request (L19811) was written to repair weld on mechanical seal injection line booster side valve.

PROBABLE CONSEQUENCES:

j At the time of the occurrence the Reactor Plant was at 33% power with the mode switch in run.

There are 4 CD/CB pumps for Unit 1 of which l

only 3 are required for 100% rated operation.

Since only the "1A" CD/CB was secured and isolated this did not effect plant operation as the other remaining pumps were operational. Thus plant operation was not J

limited.

No presence of airborne contamination was detected and all leakage was contained in the pump room and sent to Radwaste via floor drains.

CAUSE

The cause of the cracked weld was due to vibrational stress.

The condensate / condensate booster pump system contains hard piping for the mechanical seal injection line.

This was determined to be a design problem due to the vibration of the pumps causing the line to leak at welded connections.

A modification was made to the existing mechanical seal injection piping replacing existing piping with flexible high pressure hose to alleviate the vibretion problem.

The modification was in progress at the time and "lA" CD/CB was scheduled to have its mechanical seal injection line replaced when the Icak developed.

Vll. CORRECTIVE ACTION:

The "lA" mechanical seal injection line which developed the leak was replaced with the new high pressure flex hose, per modification under field change request, FCR L0197 The modification to mechanical seal injection lines for all CD/CB was completed.
