EVENT DESCRIPTION

During the performance of LTS-100-15, the Primary Containment Fersonnel Access Hatch leak rate test, both ' doors were inadvertently opened at the same time, this loss of primary containment occurred at 1515 hours0.0175 days <br />0.421 hours <br />0.0025 weeks <br />5.764575e-4 months <br /> on October 10, 1982 while the unit was running at 385 MWE.

V.

PROBABLE CONSEQUENCES OF THE OCCURRENCE:

THe drywell had a negative pressure relative to the reactor building.

The inward air flow caused the doors to open only a few inches.

No significant radiological releases were observed by the Radiation Chemistry Technician who was present when the doors were open.

The Health and Safety of the public and plant person'nel was maintained at all times.

VI.

CAUSE

Section 2.5.8 of the Instruction Manual for the airlock describes a condition called rebounding,or bounce.'From outside the Exterior door the inner door was accidentally opened first. When it was closed, the door was swung fast enough to cause the door to rebound past the point where the latch could secure the door in the closed position.

Even though the door was open the latch was in the. latched position thus allowing the exterior door to be opened.

Vll. CORRECTIVE ACTION:

The immediate action taken was to close the interior door restoring primary containment integrity; this was done in less than six minutes.

The long term corrective action is also described in section 2.5.8 of the instruction Manual for the airlock.

i Sections 2.5.8.1. and 2 5.8.2. describe a periodic inspection and l

adjustment which will assure door integrity.

The periodic inspection will be incorporated into the six month airlock door interlock surveillance.

l The interlock test was performed shortly after the event and was success-l ful as was the leak rate test.

A Work Request has been initiated to perform

~

the bounce test during the fi'rst extended outage.
