Gentlemen:

SUBJECT: REPORT NOS. 50-259/83-18, 50-260/83-18 AND 50-296/83-18

This refers to the special safety inspection conducted by Mr. G. L. Paulk of this

office on May 2 - 6, 1983, of activities authorized by NRC License Nos. DPR-33,

DPR-52 and DPR-68 for the Browns Ferry facility. Our preliminary findings were

discussed with Mr. J. A. Coffey, Acting Plant Superintendent, at the conclusion

of the inspection. Also, an enforcement conference was held at the Browns Ferry

facility on June 1, 1983. At that meeting Mr. R. C. Lewis, Director of Project

and Resident Programs, Region II, expressed NRC concerns related to the inspec-

tion findings to Mr. H. J. Green, Director, Nuclear Power, TVA, and members of

his staff.

Areas. examined during the inspection and our findings are discussed in the

enclosed inspection report. Within these areas, the inspection consisted of

selective examinations of procedures and representative records, interviews with

personnel, and observations by the inspector.

During~ this inspection, it was found that certain activities under your license

appear to violate NRC requirements. We wish to emphasize the fact that we are

concerned about the lack of managerial oversight at the Browns Ferry facility

that apparently caused the violations identified in enclosure (1) to this letter.

These violations relate tc the recently completed defueling operation performed

on Unit 1 - an operation of safety significance that had been properly performed

on several previous occasions.

It is to be noted that Item A in enclosure (1)

was considered for escalated enforcement action.

However, because records

existed to document that quality assurance tracking and control had been utilized

to verify the presence of neutron absorbers in the spent fuel racks, we conclude

that the safety significance of the event does not warrant civil penalty action.

This conclusion is supported by the results of neutron source testing of the

racks, which was conducted in June, 1983.

Based on a conversation with Mr. H. J. Green, Director,-Nuclear Power, on

June 22, 1983, I understand that the next refueling, scheduled for September 1983

'

will not commence without the concurrence of this office. Our concurrence will

'be based on the results of a special inspection designed to provide additional

assurance that corrective actions have been fully implemented to ensure that

refueling activities can be safely conducted.

'

1.

Improvements made in control systems to assure commitments made to the

NRC are fully met

2.-

Modifications made in QA and QC programs to assure readiness to

undertake major safety-related activities like refueling operations

3.

Steps taken to increase the presence of plant managers and supervisors

at the working sites to personally observe safety-related activities

and

4.

Changes made in training programs that will provide additional assur-

ances that both workers and ouality control personnel are trained to

properly perform safety-related tasks.

In accordance with 10 CFR 2.790(a), a copy of this letter, its enclosures, and

your reply will be placed in the NRC's Public Document Room upon completion of

our evaluation of the reply.

If you wish to withhold information contained in

the inspection report, please notify this office by telephone or include a

written application to withhold information contained therein, in your response.

Such application must be consistent with the requirements of 2.790(b).

The responses directed by this letter and the enclosures are not subject to the

clearance procedures of the Office of Management and Budget as required by the

Paperwork Reduction Act of 1980, PL 96-511.

Should you have any questions concerning this letter, we will be glad to discuss
