APPENDIX A Burns and Roe, Incorporated Docket No. 99900503/83-02 NOTICE OF NONCCNFORMANCE Based on the results of an NRC inspection conducted on June 6-10, 1983, it appears that certain of your activities were not conducted in accordance with NRC requirements.

A.

Burns and Roe (B&R) Topical Report (B& ROE-COM4-1-NP-2A) Chapter VII,

" Control of Purchased Material, Equipment and Services," Section 2.2 states, in part, " Source surveillance is performed by B&R, Inc. at a suppliers. facility when prepurchased items are of such a nature that compliance with procurement documents may not readily be accomplished during receiving inspection." Section 6.6 of the aformentioned document states, in part, "B&R, Inc. vendor surveillance group is responsible for:

Auditing compliance of suppliers to their approved quality assurance programs / plans. Verifying performance of selected processes, inspections and tests in accordance with specified requirements."

Contrary to the above, no approved plan has been formulated for audits of DIS /ADLPIPE, Inc., and no source surveillance has been accomplished by B&R on any of its computer program suppliers with the exception of Foster Wheeler.

B&R Topical Report (B&R0E-COM4-1-NP-2A) Chapter III, " Design Control,"

Section 5.2 states, in part, "The design verification process assures that the final design documents fully comply with the applicable design bases, regulatory requirements, codes and standards....

Design verification, except where testing is used, is completed prior.to final release _of design documents for work."

Contrary to the above, the final design of the remote shutdown panel did not fully comply with the applicable design basis nor did the design i

verification process detect this noncompliance.

A valve, required to be operable from the remote shutdown panel at WNP-2, did not exist on the panel.

Also, power to a supervisory panel in the remote shutdown panel, which is required to be independent of the control room, originated in the control room.
