BELLEFONTE NUCLEAR PLANT UNITS 1 AND 2 - FAILURE OF B.I.F. BUTTERFLY VALVE SEATS - BLRD-50-438/82-37, BLRD-50-439/83 FIFTH INTERIM REPORT The subject deficiency was initially reported to NRC-0IE Inspector R. V. Crlenjak on May 11, 1982 in accordance with 10 CFR 50.55(e) as NCR 1819 This was followed by our interim reports dated June 8, and October 29, 1982, and February 8, and May 25, 1903 Related NCRs 2170, 2186, and 2187 were included in our second interim report and liCR 2231 was included in our fourth interim report. Enclosed is our fifth interim report. We expect to submit our next report by September 18, 1984. We consider 10 CFR Part 21 applicable to this deficiency.

Description of Deficiency The following butterfly valves, manufactured by Basics Inflow Incorporated (BIF) a unit of General Signal Corporation, West W' rwicke, Rhode Island, a
which are used in the Essential Air System were determined to be leaking in excess of an acceptable rate.

- damage, i.e., the seats were cut and portions were missing. The cause of the damage was not apparent.
Interim Progress TVA has received the vendor's " proof-of-design" test report which indicates that the valves will give acceptable service for the intended application.
The vendor also provided recommendations for long-torm storage of the valves to maximize valve seat shelf life. TVA is currently reviewing these recom6endations to determine what impact, if any, seat degradation had on the failures documented. TVA will provide a final report upon determination of the root cause of the deficiency and the appropriate actions required to prevent recurrence.
