-APPENDIX A NOT7.CE OF VIOLATION Philadelphia Electric Company Docket Nos. 50-277, 50-278 Peach Bottom Atomic Power Station License Nos. DPR-44, DPR-56 Units 2 and 3 As a result of the inspection conducted on April 18-22, 1983, and in accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C) published in the Federal Register on March 9, 1982 (47 FR 9987), the following violation was identified.
10 CFR 50, Appendix B, Criterion V, and the Peach Bottom Quality Assurance Plan, Volume III, Section 5.0, require that activities affecting quality be accomplished in accordance with documented instructions and procedures.
Contrary to the above, as of April 22, 1983, activities affecting quality were not accomplished in accordance with documented instructions and procedures as evidenced in the following examples:
(1) Licensee procedure A-42, Jumper Log Procedure, requires accurate records be maintained of jumper installations, however, an accurate record was not maintained in that jumper number 4-5 (1982) was logged in the Unit 3 log when actually installed in a Unit 2 cabinet and was not cleared from the log when removed from the cabinet.
i (2) The licensee Quality Assurance Plan, section 6.2, requires the use of current procedures when performing activities, however a superceded revision of Surveillance Test 12.1A was used when the surveillance was performed on February 20, 1983.
This is a Severity Level V violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, Philadelphia Electric Company, is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice a written statement or explanation in reply, l
including: (1) the corrective steps which have been taken and the results l
achieved; (2) the corrective steps which will be taken to avoid further viola-tions; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
