BELLEFONTE NUCLEAR PLANT UNITS 1 AND 2 - QA TEST DOCUMENTATION ON RADIATION MONITORING EQUIPMENT - BLRD-50-438/82-62, BLRD-50-439/82 FOURTH INTERIM REPORT The subject deficiency was initially reported to NRC-0IE Inspector F. Cantrell on August 24, 1982 in accordance with 10 CFR 50.55(e) as Audit 82V-29, Deficiencies 3 and 4.

This was followed by our interim reports dated September 22, and November 9, 1982, and January 10, 1983 Enclosed is our fourth interim report. FOURTH INTERIM REPORT Description of Deficiencies During th'e TVA audit of GA Technologies, Inc. (formerly General Atomic Company), San Diego, California,:the following deficiencies were noted in the implementation of their previously accepted QA program for TVA contracts:

Deficiency 3 Contrary to section 11, paragraph 5 3, of GA Technologies' QA manual, a test operator did not completely record the test results for a radiation monitor (TVA item 03513501-001) already shipped to TVA for use at Bellefonte.

Deficiency 4 Contrary to ANSI N45.2, section 18, which requires results of inspections to be identifiable and retrievable from records storage, QA records from GA Technologies' Cimex facility do not provide for the required identification and retrievability of inspection results.

GA Technologies has also supplied radiation monitoring equipment for other TVA nuclear plants.

Interim Progress Concerning incomplete test results, GA Technologies has examined their documentation, and having determined this to be an isolated case, will field test the one monitor with incomplete test documentation at a date still to be determined. To prevent recurrence of this problem, GA Technologies has rewritten their test travelers to provide for this activity as a separate traveler operation.

Concerning the QA recorda identified in deficiency 4, GA Technologies has performed training for the Cimex QC inspector and is properly maintaining their records. Also, the batch numbering practice is satisfactory.

However, a statement by G. A. Technologies that missing traveler package documentation noted during the audit was a random error was found to be incorrect. Subsequently, GA Technologies has been requested to provide definite corrective action and recurrence control for this problem.
