Portz In order to implement the requirements of Information Notice 80-06, Information Notice 80-06 Supplement 1, NUREG 0654, IE Bulletin 80-15, and the DAEC Emergency Plan, the following provisions shall be implemented during Unusual Events, Alerts, Site Emergencies, and General Emergencies as defined in the DAEC Emergency Plan and significant events as defined in Information Notice 80-06.

This document is not intended to replace the existing DAEC Emergency Plan Implementing Procedures (EPIPs). This' Special Order addresses notification (communi-cation) requirements only and is intended only to help summarize the notification requirements of the above listed documents.

within one hour of the occurrence of the event. This tabulation also includes a ." Sequence of Notification Number" which refers to the requirements and sequence of notification of IELPC corporate management personnel, and state,and local emergency operations. centers (E0Cs). Each " Sequence of Notification Number"

is defined in Attachment 1.

Items 1 thru 4 below require one hour notification and the establishment of an open, continuous connunication path with the NRC Operations Center (red NRC phone). The eight other Items (and Item 1 " Unusual Events" as explained below) require one hour notifi:ation only. When continuous consnunication is required, the channel shall remain open until ilRC authorized closure.

Those significant events requiring prompt notification to meet the intent of Information Notice 80-06 and/or the DAEC Emergency Plan are as follows:

1. Any event requiring initiation of the licensee's Sequence II emergency plan or any section of that plan (ie.

9. Any fatality or serious injury occurring on the Sequence I site and requiring transport to an offsite medical fac-ility for treatment. Serious injury is considered to be any injury that in the judgement of the licensee representative will require admission of the injured individual to a hospital for treatment or observation ~ ~ ~

In general, the SSE shall be responsible for initial prompt notification Lof the NRC Operations Center, IELPC management personnel, State of Iowa author- .

ities, and local authorities (Benton County and Linn County) except as specif-ically allowed in Attachment 1. The SSE shall ensure that entries into the SSE log book are made documenting a brief description of the occurrence, the in--

dividual/ agency notified, and the time of the notification for all the notifi-cz.tions as . described in Attachment 1.

Upon occurrence of one of the 12 events tabulated above, the on-duty SSE shall notify the Operations Supervisor and Assistant Chief Engineer-0perations, or Assistant Chief Engineer-Technical Support. If these individuals are not available, notify the duty Emergency / Security Directors. An Assistant Chief Engineer or a Duty Emergency / Security Director shall notify the NRC Operations Center in Bethesda, Md. via the special flRC phone in the control room witt.in one hour and shall establish an open, continuous channel of communication for Item 1 thru 4 events (except Item 1 " Unusual Events") tabulated above. If the Assistant Chief Engineers and Duty Emergency / Security Directors have not been notified and assumed responsibility for NRC notification within one hour, the on-duty SSE shall ensure all notification requirements are met.

In addition, in the event that one or more extensions of the NRC emergency notification system telephones at DAEC is found to be inoperable for any reason, the NRC Operations Center in Bethesda, Md. shall be notifiei by iommercial telephone or relayed message within one hour of the time of the discovery.

The responsibility for this notification shall be identical to the NRC Opera-1 tions Center notification requirements for any of the 12 events tabulated i

above. This requirement is imposed to fulfill the IE Bulletin 80-15 notifi-cation requirements.

Center. Content and wording guidelines are presented in Attachment 3 for tele-phone reporting to State of Iowa and local officials of Unusual Events, Alerts, Site Emergencies, and General Emergencies (Itam 1 in the above tabulation). In ,

order to simplify the notification process to state and local authorities, the r

Attachment 2 events have been assigned a coded number. The Emergency Action Level categories of events (ie. Unusual Events, Alerts, Site Emergencies, and General Emergencies) have been assigned letters A through D. Thus an event reported as '

a type "A-9 Event" would indicate to the offsite agency that an Unusual Event has occurred and that this event consisted of the lifting of a safety valve. '

In all cases, the state or local agency will contact the DAEC Shift Super-vising Engincer after receipt of a DAEC notificatirn call to ccnfirm that the i notification was valid. It is desirable that both the initial notification call and the confirmation call be made by telephone rather than by radio, if possible.

During off-duty hours (1700 hrs to 0800 hrs), if the event is classified according to Item 1 " Unusual Events" and Items 5 thru 12 tabulated above, the >

SSE shall notify the duty Emergency / Security Directors in accordance with the f procedure outlined above. The need for the duty Emergency / Security Directors to proceed to the site shall be determined through consultation with the on-duty i SSE. If it is determined that the duty Emergency / Security Directors are not needed on-site, the on-duty SSE shall make the required notifications to the NRC Operations Center, IELPC corporate management personnel, State of Iowa E0C and County E0C's as directed in Attachment 1.  ;

All Emergency / Security Directors shall be familiar with the Modified Amended Security Plan and Procedures, the Emergency Plan and Procedures, and this order.

Unusual Events - Type "A" Event 1.

Upon occurrence of an Unusual Event at DAEC as tabulated in Attachment 1, the State of Iowa, Linn County and Benton County authorities shall be notified by telephone as soon as possible. The telephone notification should be made as follows:

"This is (caller's name and job position) calling from the Duane Arnold Energy Ceni.er. An event categorized as a Type (coded event description A- ) Event has occurred at DAEC. This telephone call is being made to comply with flRC notification regulations only and no assistance or other action by your agency is required."

[ Note that only the coded event description as provided in Attachment 1 should be used for notification of Type "A" events to state and local authorities.

Following initial notification of the offsite agency, the individual making the notification should expect a verification telephcne call back from the agency to confirm the individual's identity. Thus, it 1s very important that the noti-fying individual clearly provide his or her name and job position at DAEC on the initial telephone call. Should any additional infonnation be required by the offsite agency for a type "A" event, a narrative description may be provided to appropriate personnel in the offsite agency upon request (preferrably on a return call basis) only.]

2. Alerts - Type "B" Events The telephone notification of an Alert, as tabulated in Attachment 1, to State of Iowa and Local authorities should be made as follows:

"This is (caller's name and job position) calling from the Duane Arnold Energy Center. An event categorized as a Type (coded event description B- ) Event has occurred at DAEC. This telephone call is being made to comply with NRC notification regulations only and no assistance or other action by your agency is required at this time."

3. Site Emergencies - Type "C" Events The telephone notification of a Site Emergency, as tabulated in Attachment 1, to State of Iowa and local authorities should be made as follows:

"This is (caller's name and title) calling from the Duane Arnold Energy Center. A Site Emergency categorized as a Type (coded event description C-

4. General Emergencies - Type "D" Events The telephone notification of a General Emergency as tabulated in Attachment 1, to State of Iowa and local authorities should be made as follows:

"This is (caller's name and title) calling from the Duane Arnold Energy Center. A General Emergency categorized as a Type (coded event' description D- ) Event has been declared at DAEC. [ The individual making the notification shall briefly describe the event in-cluding any details pertinent to the agency being notified. The event description shall be sufficiently detailed so as not to cause unnecessary concern or panic and yet initiate the necessary offsite support activities. The caller shall recomend appropriate actions to be taken by the outside agency including mobilization of emergency personnel, dispatching of offsite radiation monitoring teams, and associ-ated comunication activities. Projected exposure dose to the public shall be pro-vided as available. Following the initial notification of the offsite agency, the individual making the notification should expect a verification telephone call back from the agency to confirm the individual's identity. Each licensee shall inmediately notify by telephone and telegraph, mail-gram, or facsimile, the Director of the appropriate NRC Regional Office listed in Append.x D of any incident involving byproduct, source, or special nuclear material possessed by him and which may have caused or threatens to cause:

(1) Exposure of the whole body of any individual to 25 rems or more of radiation; exposure of the skin of the whole body of any indivMual of .

(c) Any report filed with the Commission pursuant to this section shall be prepared so that names of individuals who have received exposure to radiation will be stated in a separate part of the report.
