Gentlemen:
Subject: Combined Inspection 50-277/90-01; 50-278/90-01
This letter transmits the findings of the routine resident safety inspection
by Messrs. J. J. Lyash, R. J. Urban, L. E. Myers and D. Moy from January 1
through February 20, 1990, at the Peach Bottom Atomic Power Station Delta,
Pennsylvania. Direct observation of activities, interviews, and document
reviews form the basis for these findings.
The inspectors discussed the
details of the inspection with Mr. J. F. Frant of your staff.
Based on the results of this inspection it appears that some of your
activities were not conducted in full compliance with NRC requirements. One
violation for failure to adhere to the provisions of a radiation work permit
resulted in an unplanned personnel radiation exposure.
This incident was
self-identified and aggressively addressed by plant management. We have
determined that this example satisfies the criteria for licensee identified
violations as stated in 10 CFR 2, Appendix C, Section V.G.1, and as such no
Notice of Violation (NOV) will be issued.
The inspection identified one additional violation of NRC requirements for
failure to comply m i th Technical Specification (TS) 4.5.E.2, regarding
l
performance of re: tred surveillance tests with an inoperable ADS valve. The
NOV enclosed as 2'pendix A describes the specific violation. The severity
level was chart,terized according to the " General Statement of Policy and
Procedure for hRC Enforcement Actions," 10 CFR 2, Appendix C (Enforcement
Policy).
In responding, please follow the instructions contained in the
Appendix.
It appears that inadequate review and disposition of deficient surveillance
test r.sults led to the failure to implement the TS required actions.
This
review involved both technical and operations staff and supervision.
In
responding to the violation please include your corrective actions to address
this area.
