The violation involves three examples of personnel not following
radiolo!ogical centrolled zone without performing the required personnelThe first exam

Although this latter example, failure to use an adequate radiation work permit,

was also identified by your staff, we have, after careful review, elected to
l
include this example in the enclosed Notice of Violation.
The bases for our
l
decision is discussed in the enclosed inspection report. We are particularly
l
concerned that the cause.of this latter example was apparently inadequate
,
planning and review of a work activity. The need to improve planning, review
l
and control of activities that could result in the loss of control of
I
radioactive material was discussed in our August 31, 1989 letter to you.
Our inspection also identified a number of concerns in the area of contamination
control and definition of the responsibilities of your radiological controls
organization.
These concerns are discussed in the enclosed inspection report.
