APPENDIX A NOTICE OF VIOLATION
[
Philadelphia Electric Company Docket No. 50-278 Peach Bottom Atomic Power Station License No. DPR-56 Unit 3

During an NRC inspection conducted from November 20 to December 31, 1989, a violation of-NRC requirements was identified.
In accordance with the NRC Enforcement Policy (10 CFR Part 2, Appendix C), the particular violation is set forth below:
Technical Specification section 6.8.1 requires that written procedures be established, implemented, and maintained that meet the requirements of ANSI N18.7-1972, Sections 5.1 and 5.3.
ANSI N18.7-1972, Section 5.3 states that startup procedures shall be provided, and that they include control rod withdrawal sequences.
Section 5.1 requires that procedures be present and followed step-by-step.
General Operating Procedures GP-2, Revision 54, " Normal Plant Startup,"
and GP-3, Revision 44,." Normal Plant Shutdown," include detailed control rod withdrawal and insertion sequences and require that control rods be manipulated only in strict accordance with these sequences.
Contrary to the above, on 4 occasions between November 24 and Deceniber 11, 1989,. control rod manipulations were performed which were not conducted in
-accordance with Procedures GP-2 and GP-3. Specifically:
1.
On November 24, 1989, control rod 22-59 was withdrawn to notch 08 instead of 04, contrary to Procedure GP-2-3, Appendix 1, Revision 4, "Startup Rod Withdraw Sequence Instructions."
A rod worth minimizer (RWM) block prevented attempts to withdraw ths. rod further.
2.
On November 24, 1989, a control rod to be withdrawn to position 02 was left at position 00, contrary to Procedure GP-2-3, Appendix 1, Revision 4, "Startup Rod Withdraw Sequence Instructions." A RWM block prevented attempts to withdraw the next rod.
3.
On November 25, 1989, control rods 14-11 and 46-51 were inserted beyond the group notch insertion limit of 12 to notch position 00, contrary to Procedure GP-3-3, Appendix 1, Revision 1, " Shutdown Rod Insertion Sequence Instructions." A RWM insert block prevented attempts to insert additional rods beyond their insertion limits.

On December-11, 1989, control rod withdrawals were conducted on Unit 3 for at least 2. shifts utilizing Unit 2 Procedure GP-2-2,'rather than required Unit 3 Procedure GP-2-3, Revision 4, "Startup Rod Withdraw Sequence Instructions." Control rod withdrawal beyond Group 39 was prevented by a RWM withdrawal block.
The withdrawal sequences for both units were identical until Group 40.
This is a Severity Level IV violation (Supplement' I).
