Subject: Combined Inspection Report 50-277/89-26; 50-278/89-26.

This-letter transmits the findings of the routine resident safety inspection by

,

Messrs. J. J. Lyash, L. E. Myers, R. J. Urban and others on November 20 to

December 31, 1989, at the Peach Bottom Atomic Power Station, Delta, Pennsylvania.

.These findings were ba' sed on observation of activities, interviews, and document

reviews, and have been di> cussed with Mr. J. F. Franz of your staff.

During the inspection period Unit 2 continued to operate, and the extended Unit.3

outage was concluded. The management review and self-assessment efforts applied

.

to the Unit 3 restart and pcwer ascension demonstrated a commendable safety

a

perspective. We encourage your continued efforts to implement an effective

self-assessment program.

Based on the results of this inspection it appears that some of your activities

~

were not conducted in full compliance with NRC requirements. One violation for

failure to properly document Quality Control inspections was identified. .Our

review determined that this example satisfies the criteria for licensee identi-

fied violations as stated in 10 CFR 2, ' Appendix C,Section V.G.1, and as such

no Notice of Violation will be issued.

A second violation-was identified as set forth in the Notice of Violation

. enclosed as' Appendix A.

Several incidents involving failure of licensed

operators'to adhere to preapproved control rod manipulation sequences occurred.

~ As noted in the cover letter of In;pection Report 50-277/50-278/89-23 we are

cnneerned that incidents of this type may be indicative of underlying personnel

performance weaknesses. Significant management action to analyze and address

,~

I

these errors was evident during the inspection period.

Because review and

corrective action efforts undertaken appear well focused, no written response

to this violation is required.

We will review your final root cause analysis

and will continue to monitor this area closely. Your continued careful atten-

tion is warranted to ensure that the underlying root causes are identified and

addressed.
