Results: Based on the grading of the reactor operator's written retake examination, the reactor operator passed the examination.

The reactor operator's file will be updated to reflect the satisfactory passing grade for the examination.

Examination 1

The examination was conducted to reexamine a' Reactor Operator in' the written portion of the licensed operator requalification program evaluation. The Reactor Operator had been previously evaluated as unsatisfactory, by both the licensee and the NRC, in-the written portion of the NRC evaluation of the San Onofre Nuclear Generating Station Units 2 and 3 licensed operator requalific6 tion program. The results of the previous examination are documented in Examination Report l

The licensed operator requalification written examination was administered and developed in accordance with Examiner Standard ES-601, Revision 5, Administration of NRC requalification Program Evaluations.

Examination Development The written examination questions and reference material were. reviewed by the Chief Examiner in accordance with ES-601. The Chief Examiner commented on the examination questions provided for examination development. The coments were resolved-with the licensee representatives and the examination was finalized. The licensee representatives, who participated in the examination review and preparation, signed examination security agreements as required by ES-601.

L During the review of the written examination bank questions, provided by the licensee for this examination, the Chief Examiner noted that the majority of the questions in the question banks were not objective, and did not always use the guidance of ES-601, Attachment 14, " Guidelines for the Development and Review of Open Reference-Examinations." The licensee representatives stated that the examination question banks would be reviewed and questions would be revised and developed to. meet more l

objective grading criteria and standards.

During the review of thE facility reference material, the NRC examiner noted a discrepancy in plant procedure 01 S023-6-15. " Operation of 125 VDC systems," page 4 of 32, Precautions and Limitations, paragraphs 4.5.

and 4.7.

The discrepancy was in the conversion of degrees Fahrenheit to degrees Celsius. The temperatures in question related to the maximum allowable operating conditions for' the'125 VDC equipment as defined in its environnental-qualifications. The licensee agreed to look-into making a revision to the precautions and limitations for this equipment.
