APPENDIX A NOTICE OF VIOLATION Public Service Electric and Gas Company Docket Nos. 50-272, 50-311 Salen Generating Station, Units 1 and 2 License Nos. DPR-70, DPR-75 As a result of the in?pection conducted on July 1 through August 3,1981 and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:

A.

Technical Specification 6.8.1 states in part:

" Written proce'dures shall be established, implemented and maintained covering... the applicable procedures recorxiended in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978." Regulatory Guide 1.33 included Paragraphs A.3 Equipment Operation (e.g. locking). The inplementing procedure, Operations Directive Manual. Fevision 6, states, in Section 2.6.2, "...When a valve lineup is perfomed, the operator is to position all valves as specified by the valve lineup and initial the appropriate space... The completed valve lineup will then be placed in the valve lineup book at the operator's desk...

When a valve lineup is deviated from due to tagging or special plant conditions, the control operator will note in the "C&T For or Out of Position Column:

the reason..."

Contrary to the above, approximately 3:00 p.m. on July 16, 1981, manual isolation valve 23MS45 was found unlocked in the open

. position. This valvo is required by operating procedures to be locked open in order to provide redundant steam supply to No. 23 Auxiliary Feed Pump. No deviation or reason for the unlocked valve was noted in the valve lineup list..

This is a Severity Level V Violation (Supplement I). (Unit 2 only)

10 CFR 20.203(b) states in part, "Each radiation area shal.1 be conspicuously posted with a sign or signs bearing the radiation caution symbol and the words:

Caution ibdiation Area or Danger Radiation Area Contrary to the above, approximately 2:30 p.m. on July 14, 1981, the floor plug (6'x8') had been removed above No.11 RHR pump pemitting access to the pump. Review of the radiation survey, map 13, dated July 14, 1981, indicated 40 - 60 mrem /hr around the pump. No radiation sign had been posted at this access when the floor plug was removed.
