Appendix A NOTICE OF VIOLATION Commonwealth Edison Company Docket No. 50-295 50-304 As a result of the inspection conducted on August 1, 1981 to October 1, 1981 and in accordance with the Interim Enforcement policy, 45FR6754 (October 7,1980),
the following violation was identified:
The code of Federal Regulations 10CFR 50.59 Section (b) states in part that the licensee shall maintain records which include a written safety evaluation which provides the basis for the determination that a change to the facility does not. involve an unreviewed safety question.
Contrary to the above, between September 16, 1980 and January 23, 1981 the licensee performed a modification to the control circuitry of eight containment isolation valves without a written safety evaluation. This is a severity level IV violation.
Pursuant to the provisions of 10CFR 2.201 you are required to submit to this office within twenty-five days of the date of this notice a written statement or explanation including for each item of noncompliance:
(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved.
Under the authority of Section 182 of the Atomic E.tergy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
