It has been determined that 4 liquid vaste releases (approxicately 120,000 gals) were cade without adequate saapling having been conducted.

This was the result of a change having bein nade to the system configur-ation without a corresponding ch'.nge being made to the sample point.

The releases (approxirately 30,000 gals. each) were cade te 0557 on 10-19-81,1953 on 10-19-81,1115 on 10-20-81, and 2214 on 10-20-81. The releases were made with " inoperable" discharge nonitors which requires redundant sa1pling per Tech. Spee. 3.9.7.

The discharge monitors were "in service" but were administratively inoperable as a result of an inability to meet correlation requiresents. Based on an evaluation of the discharge nonitor data and other sample results, the isntopic contents of the 4 relpsea has been conservatively estinatt.d and NO LIMITS vere exceeded. This is considered to be a violation of Tech. Spec. 3.9.7 and
